Ala. Vols, was 19 years old, blue eyes, black hair, fair complexion, 5 feet 10 inches high, born in Randolph county in the states of Alabama by (blank) former (blank) was enlisted on the 5th day of 1862, by Co. C. M Shelley to serve 3 years or during the war. The said Joshua Horn was last paid by Capt E Johnson (blank), on the 31st day of August 1862, and his pay due him from that time to the time of his death, which occured on the 22nd day of May 1863. For pay from the 1st of Sept 1862 to the 22nd day of May 1863, being eight months and 22 days at $11.00 per month. For accumulation for clothing not drawn from 1st Sept 1863(sic) to the time of his death. Amount (blank) He is endebted to the C.
