That at his request I have made an examination of physician(sic) condition and find: (State description and character of wound) NONE Physical Condition and to what cause is his incapacity for manual labor attributable To Paralysis Agitans, Rheumatism, Hemmoroids which (unreadable) applicant wholly diasabled for manual labor. His age is 71 years and that said disability is not the result of his own habits still persisted in IT IS NOT Extent of disability WHOLLY DISABLED AS RESULTS MENTIONED ABOVE.
