The doctor attended Mr. Givens from July 7, 1957 until Oct. 30, 1957.

Additional Comments:
The copy from the microfilm was very difficult to read.  The cert. # was 
listed on an index as 20241 (Vol. 41, Roll 5)

I believe this to be Elisha Porter Givens who m. Ella Morgan 1904 in Houston 
Co., AL.  Ella's first name may have been Mary. 

Per census records Elisha was born abt. 1880.

I am researching this family and would like to correspond with anyone who may 
have more info on this family.
