The State of Alabama
Montgomery County

  Before me Clyde L. Ellis a notary public in and for said State and county, 
personally appeared Mrs. N. J. Simmons being by me first duly sworn, deposes 
and says that she is a resident of Lee County, Alabama, her post office being 
Phenix City, that she is the widow of T. S. Simmons who served in the 
Confederate Army as Corporal in Co. A being in the 63 Regiment , that she was 
born at or near Covington in the County of Newton state of Georgia on the 14 
day of July 1839 being 76 years of age at the time of making this affidavit: 
that she is now on the pension roll of Lee County under number 36730
Affiant further states that she hereby makes application under an act approved 
March 8, 1915, to be placed on the reclassified roll.

Sworn to and subscribed before me this 20 daky of April 1915,
signed by N.J. Simmons and Clyde L. Ellis Notary Public
