To whom it may concern:  This is to verify that there is a record in this 
office of the death of HENRY BAKER, white male, who expired at the age of 48 
years, on June 19, 1896. He was born in Alabama. The cause of death is listed 
as "Phehises." He died and was buried in Wheelerville, Alabama. The attendant 
wass D.O. McCrary.
