The Church at Wesley's Chapel
This the 6th day of May 1894  accept the Union with the Congregational and 
Methodist Protestant Church upon the bases agreed upon by the convention of 
said Church held on the 13th day of Oct 1889 in [Athens or Atlanta] GA. 
Resolve 1st that we agree to accept the name Union and call our selves by that 
name a United Congregational Church Reselove second that we accept constution 
by Laws us one constution and by Law and Goverment as agreed upon by the said 
convention this dun by us or as many of us has as has annxt our names above on 
this above day & date
