Dortch aged 57 years, a resident of Ada, Conway Co., ARk. whose Postoffice address is Ada, Ark. who, being duly sworn according to law, declares taht he is the Olsey C. Dortch who was ENROLLED on the 24 day of November, 1863, in Co. E, 4th Ark. Cav. in the Service of the United States, in the war of the Rebellion, and served at least ninety days, and was Honorably Discharged at Little Rock on the 28 day of March 1864, and who is an applicant for Invalid Pension on accounty of Broken leg. (2) That in addition to the named disabilities heretofore alledged, he is disabled for manual labor by back complaint and impaired eyesight. (3) He further declares that said disabilities render him unable to earn a support by manual labor; that none of said disabilities are in any way due to vicious habits, and are, to the best of his knowledge and belief, of a permanent character; and that he has not been in either the military or naval service of the United States, except as above set forth. that he makes this supplemental declaration for the purpose of reoeing his claim, and being placed on the Pension Roll of the United States under the provisions of teh Act of June 27, 1890. He hereby appoints Cos. Altenberg, of Little Rock, Arkansas, with full power of substitution and revocation, his true and lawful attorney to prosecute his claim, and hereby agrees to pay his said attorney the legal fee of TEN DOLLARS when the claim is allowed. I object to being examined before teh Board at LIttle Rock, Ark. because as I am informed & believe said Board is prejudiced against Ex-Federal Soldiers & will not give me justice. D., 1902, personally appeared before me, a Justice of the Peace, in and for aforesaid county, duly authorized to administer oaths G. M. Miller, aged 53 years, whose P. O. address is Ada, Ark., and G. W. Hopkins, aged 35 years, whose P. O. address is Ada, Ark., well known to me to be reputable and entitled to credit, and who being duly sworn declares in relation to the aforesaid case as follows: That we have known the Claimant Olsey C. Dortch for thirty-five and twenty years, respectively. That claimant has been during our acquaintance with him a sober, perservable man of good habits. From our acquaintance with him we are satisfied that his injuries were not caused by vicious habits. That we have no interest, either direct or indirect in the prosecution of this claim. Attest: J. D., 1907, personally appreard before me a Justice of the Peace within and for the county and state aforesaid Olsey C. Dortch, a resident of Adona, County of Perry, State of Arkansas, who after being duly sworn according to law declares that he is the identical Olsey C. Dortch who was enrolled as a private soldier on the 24 day of November 1863 in and for Company E 4th Reg't Arkansas Cav, in the Civil War of 1861 and 1865, and that he served upwards of ninety days, and was honorably discharged from said service on the 28 day of March, 1864. That he makes the application for the purpose of being placed on the pension rolls of the United States Government under the pension service bill, approved February 7, 1907. That he is a pensioner of the United States, enrolled at the Knoxville agency, at the rate of Eight dollars per month, by pension certificate no. 1060731. That he is 67 years of age, having been born on the 20th day of March, 1839. That he calls special attention to evidence on file by him in the prosecution of his claim for invalid pension, as to his correct age, and his military service, corroborative to this, his claim for service pension, and asks that same be considered satifactory. Dortch ----------------------------------------------------------------------------- DECLARATION FOR PENSION Act of May 1, 1920 The Pension Certificate Should Not Be Forwarded With the Application State of Arkansas, County of Conway ss: On this 21 day of June 1922, personally appeared before me, a Notary Public within and for the county and State aforesaid, Olsey C. Dortch who, being duly sworn according to law, declares that he is 83, years of age, and a resident of Morrilton, county of Conway, State of Arkansas; and that he is the identical person who was enrolled at Benton, Arkansas, under the name of Olsey C. Dortch on teh 24 day of November, 1863, as a private in Co. E., 4 Regiment of Arkansas Calvary (Col. Fishback) in the service of the United States, in the Civil War and was honorably discharged at Benton, Arkansas, on the 28 day of March 1864. That he also served (blank space). That his personal description at enlistment was as follows: Height 5 feet 7 inches; complexion light; color of eyes blue; color of hair light; that his occupation was farmer; that he was born March 20, 1839, at near Benton, Saline Co., Ark. That he requires the regular personal aid and attendance of another person on account of the following disabilities: General nervous breakdown-Rheumatism renders him unable to get about without assistance. That since leaving the service he has resided at Benton, Ark, Little Rock, Ark., Wilburton, Okla. and Ada and Morrilton, Arkansas. and his occupation has been farmer. That he has applied for pension under original No...... That he is a pensioner under Certificate No. 1060731. That he maked this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 1, 1920.
