Enlisted: Little Rock, Ark., December 14, 1917.
Died: Naval station, Great Lakes, Ill., January 28, 1918.
Cause: Pneumonia, broncho.
Next of kin: Father, L. N. Cook, R. F. D. 1, Mount Morian, Ark.
