D. 1891, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths Virgil W. Earp aged 46 years, late Pvt, Co. C, 83rd Reg't. of Ills Inft. Vols., applicant for original pension Jun. 27th, Pension No. 954.186 a resident of Colton County of San Bernardino, State of Cal whose Postoffice address is San Bernardino (?), San Bernardino Cal and well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows; That he claims pension under the NEW LAW on the following disabilities: FIRST Gun Shot Wound in right leg (indecipherable) at or near Tombstone State of Arizona Ter. on or about 26th of Oct. year of 1881 Under the following circumstances: Fight with Stage Robbers SECOND Shot in left arm contracted at or near Tombstone Arizona Ter. on or about 28th December year of 1881 Under the following circumstances: Shot by Stage Robbers THIRD Shot in Back at or near Tombstone State of Arizona on or about 28th of December year of 1881 Under the following circumstances: Shot by Stage Robbers FOURTH Neuralgia of Stomach contracted at or near Colton State of California on or about 1888-89 year of . . . Under the following circumstances: Caused by wounds in back. That he rendered military service as follows: All the service I rendered was in Co. C 83rd Illinois Infantry. That the date of his final discharge from the service was on the 4th day of July 1865. That he has rendered no military or naval service otherwise than as above set forth. Virgil W.
