D. One Thousand Nine Hundred and twenty one, personally appeared before me, a notary public in and for the County and State aforesaid Mrs. A. N. Adams a resident of Windsor County of Alachua State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of the General Pension Laws of Florida as amended by Chapter 8400, Acts of 1921. That she is the lawful widow of William Adams who enlisted under the name of William Adams on the 16th day of May, 1862 in Company K 2d Florida Cavalry Regiment of the State of Florida and who was honorably discharged at April, 1865, on account of the close of the war. That he also served in Florida. No, he never drew a pension. That she was lawfully married to the said William Adams under the name of A. N. Thomas in the County of Alachua State of Florida on the 19th day of February, 1874, and that she was not divorced from him before nor has she remarried since his death, which occurred on the 5th day of February, 1912, in the County of Alachua, State of Florida except as hereinafter stated. That she is a resident of Alachua County, Florida, and has continuously resided in the State of Florida since the 19th day of March 18?? Cavalry Regiment of the State of Florida, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that William Adams enlisted in Company K 2 Fla. Cavalry Regiment of the State of Florida, at ____, in the State of Florida on the 16 day of May, 1862; that he served faithfully in the said organization until the ____ day of April, 1865, when he was HONORABLY DISCHARGED on account of close of the war in the State of Florida, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. (Here state whether the soldier was transferred to another organization, or not, and if captured or imprisoned, state when and where.) Was not transferred, captured or imprisoned. I know personally of the above stated service, because I served with William Adams in Company K Regiment 2d Fla. Cavalry of the State of Florida, and was HONORABLY DISCHARGED at ____, in the State of Florida, on the ____ day of May, 1865, by reason of the close of the war and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War.
