Womack a resident of High Springs County of Alachua State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of the General Pension Laws of Florida. That she is the lawful widow of W. J. Womack, also borne as Wiley J. Womack also died as Wiley Jonithan Womack who enlisted under the name of W. J. Womack also born as Wiley J. Womack on the see transcript of service attached, 1864, in Company C Thirty First from Wake County Regiment of the State of North Carolina and who was honorably discharged at see attached transcript of service. No pension drawn by husband. That she was lawfully married to the said Wiley J. Womack under the name of Minnie Amelia Thomason in the County of Wake State of North Carolina on the 10th day of November, Eighteen Hundred Sixty Nine, and that she was not divorced from him before nor has she remarried since his death, which occurred on the 25th day of May, 1915, in the County of Johnston, State of North Carolina, except as hereinafter stated. Not divorced. Not remarried since husband's death. She was borne Nov. 3, 1851 and is now 83 years of age. That she is a resident of Alachua County, Florida, and has continuously resided in the State of Florida since the ____ day of December, 1925. That she was heretofore granted a pension from the State of Florida under Certificate No. none. That she is not a pensioner of any other State. Received a pension from N. C. until Dec. 1934 Sec. 1445 reads in part as follows: *** Provided, such widow shall have resided in this State continuously for a period of eight years next preceding the date of filing of her pension claim.
