D. One Thousand Nine Hundred and thirty one personally appeared before me a notary public in and for the County and State aforesaid Mrs. Grace Albert a resident of Miami County of Dade State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of the General Pension Laws of Florida. That she is the lawful widow of John W. Albert who enlisted under the name of John W. Albert on the 7th day of September, 1863, in Company "D" 23rd Calvary Regiment of the State of Virginia and who was paroled April 12th 1865 at Winchester, Va. on account of being prisoner of war. That he also served (see attached copy letter from War Dept.) That she was lawfully married to the said John W. Albert under the name of Grace Husson in the County of Putnam State of Ohio on the 21st day of August, 1873, and that she was not divorced from him before nor has she remarried since his death, which occurred on the 25th day of August, 1900, in the County of Putnam, State of Ohio except as hereinafter stated. That she is a resident of Dade County, Florida, and has continuously resided in the State of Florida since the 5th day of September 1923. That she was heretofore granted a pension from the State of Florida under Certificate No. ____. That she is not a pensioner of any other State. That her Postoffice address is #744 West Flagler St., Miami, County of Dade, State of Florida. Mrs. Grace Albert (Signature of Claimant.) Attest: (1) W. E. Norton (2) H. S.
