One Thousand Nine Hundred and Thirteen, personally appeared before me, a Notary Public in and for the county and State aforesaid, Samuel E. Brooks, who, being duly sworn according to law, declares that he is 73 years of age, having been born on the 11th day of October, 1840, in the county of Macon, in the State of Georgia. That he is a bona fide citizen of the county of Escambia, State of Florida. That he has resided in the State of Florida continuously since the _____ day of January AD 1900. That he is the identical person who enlisted at Mobile, Alabama, under the name of Samuel E. Brooks, on the _____ day of Summer 1862, in Company "B", Regiment 17th of the State of Alabama "Inf't" in the service of the Confederate States Army (Capt. Moreland) and who was HONORABLY DISCHARGED at __________, in the State of __________, on the _____ dau of _____, 186__, on account of _End of War. (Here state fully any other military service performed by the applicant) At the Battle of Nashville Tenn, just before the surrender, I was cut off from fort of our company, did not get across the River for about 15 days, went thru to Corinth Miss and from there to Mobile where I was one of a party under Corporal Ensley--detailed to gather supplies for the hospital. While on this trip we were called into Covington County, Ala. account a Yankee Raid. at time of the surrender we were just south of Montgomery and I never received any parole or discharge, Our Capt said "Go Home." That I served faithfully until end of war in the service of the Confederate States in the year 1865 and did not desert the service of the Confederate States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War) That I was Just south of Montgomery on detail with Corporal Ensley Real estate, located at -- Own no property Disabilities--old age, physical Wounds received--wounded in left foot by bayonet in hands of Union soldier Post Office Address--Bluff Springs (RFD), County of Escambia, State of Florida Signed: Samuel E. Brooks, who is an applicant for a pension under the laws of Florida, and that from our own personal knowledge and from the best information available, we believe that the applicant does not own property (including the property of his wife) to exceed in value the sum of $5000, and that the statements made by him relative to the value of his property are true and correct. Durrett, who being duly sworn each for himself, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows Samuel E. Brooks the applicant named in the foregoing application for a pension. The deponents further say that they have carefully examined the said applicant's physical condition and find: That the applicant has chronic Bright's Disease and as a consequence has an enlarged heart. This deponent further says that the said Samuel E. Brooks is permanently and totally disabled by reason of such disease from earning a livelihood for himself by manuel labor. State of Florida} County of Escambia} Before me, a Clerk Circuit Court in and for the State and County aforesaid personally came Dan Majors of Pensacola, County of Escambia, State of Florida, who being duly sworn according to law, declares that he is the identical person who enlisted at South Butler in the State of Alabama on the _____ day of __ in 1861 in Company B--17th Regiment of the State of Alabama, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that Samuel B. [E?] Brooks enlisted in Company B-17th Regiment of the State of Alabama at Mobile in the State of Alabama on the ___ day of ___ 186__. ... [the other blanks are left blank] Signed D. State of Florida} County of Escambia} Before me, a Clerk Circuit Court in and for the State and County aforesaid, personally came Henry C. Flowers of Pensacola, County of Escambia, State of Florida, who being duly sworn according to law, declares that he is the identical person who enlisted at Mobile in the State of Alabama on the _____ day of April 1862 in Company B--17th Regiment of the State of Alabama, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that Samuel E. Brooks enlisted in Company B-17th Regiment of the State of Alabama. He was enlisted when I went in. That he served faithfully in the said organization until the ___ day of ___ 1865, when he was honorably discharged ON ACCOUNT OF SURRENDER, AT ... I know personally of the above stated service because I served with Samuel E. Brooks in Co. B--17th Regiment of the State of Alabama, and was HONORABLY DISCHARGED at Montgomery in the State of Alabama on the ___ day of ___, 1865 by reason of surrender, and I did not desert the service of the Confederate service nor take the oath of allegiance to the United States before the close of the war. Brooks, private, Company B, 17th Alabama Infantry, C. S. A. enlisted September 6, 1862, at South Butler, Alabama. On the roll of the company for November and December, 1862, last on file, he was reported present, and his name appears on a register of Confederate soldiers who were paroled June 10, 1865, at Montgomery, Alabama. Brooks of Los Angeles California, who evidently had written the State Board of Pensions to ask about the family and pension record of Samuel E. Brooks (apparently for genealogy reasons).
