(FORM A) State of Florida,} County of Hardee} On this 7th day of Aug., A. D. One Thousand Nine Hundred and ninety-three personally appeared before me, W. R. Minor, notary public in and for the county and State aforesaid, W. H. Culpepper, who, being duly sworn according to law, declares that he is 78 years of age, having been born on the 14 day of Feb., 1845, in the county of Warren Co., in the State of Ga. That he is a bona fide citizen of the county of 1910 [sic] , State of Florida. That he has resided in the State of Florida continuously since the ____ day of Oct., 1910. That he is the identical person who enlisted at Savannah, Ga., under the name of W. H. Culpepper, on the ____ day of ____, 1863, in Company F, Regiment 20th Ga. Battalion of the State of Ga. in the service of the Confederate Army and who was HONOARABLY DISCHARGED at Augusta, in the State of Ga., on the ____ day of ____, 1865, on account of surrender. (Here state fully any other military service performed by applicant.) Enlisted at Sapalo, Ga. 1861 Co. E 29th Ga. Reg. under Capt. Bill Young - discharged underage 1862 at Savannah. Re-enlisted 4th Ga. Cavalry Co. F or Capt. ? Co. Transferred from 4th Ga. Cavalry to 20th Ga. Battalion Co. F. 1863 which I served with until the surrender at Augusta, Ga. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate Army in the year 1865, and did not desert the service of the Confederate Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was Augusta, Ga. on detail duty. That I have not applied for a pension from the State of Florida and refer to Claim No. ____ for the proof contained therein. Battalion Co. F Regiment of the State of Ga., which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that W. H. Culpepper enlisted in Company F 20th Ga. Battalion Regiment of the State of Ga., at Savannah, in the State of Ga. on the ____ day of ____, 1863; that he served faithfully in the said organization until the ____ day of surrender, 186_, when he was HONORABLY DISCHARGED on account of surrender, at Augusta in the State of Ga., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. I know personally of the above stated service, because I served with W. H. Culpepper in Company F Regiment 20th Ga. Battalion of the State of Ga., and was HONORABLY DISCHARGED at Augusta, GA. at surrender, in the State of Ga., on the ____ day of ____, 1865, by reason of surrender and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. Bat of the State of Ga., which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that W. H. Culpepper enlisted in Company 20th Ga. Battalion Co F. Regiment of the State of Ga., at Savannah, in the State of Ga. on the ____ day of ____, 1863; that he served faithfully in the said organization until the surrender, 186_, when he was HONORABLY DISCHARGED on account at Augusta surrender [sic], at Augusta in the State of Ga., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. The name W. H. Culpepper has not been found on the rolls of Co. F, 20th Battn. Ga. Cav., CSA, nor on rolls of any other Co. of said organization. The records show, however, that one, W. H. Culpepper enlisted Dec. 10, 1862 at Wayneville as a private in Co. F, 4th (Clinch's) Ga. Cav'y, CSA, and was transferred to 20th Battn Ga. Cav. Jan 1, 1863. Name was taken up on rolls of Co. F, 20th Battn, GA. Cav. as William Culpepper and is reported on roll of said Co. for Sept & Oct, 1864, (last on file), private, present. This Co. subsequently became a part of the 10th Ga. Cav. by reasons of consolidation, but the name of said soldier does not appear on the rolls of any Co. of that organization. The prisoner of war records show, however, that one, W. H. Culpepper, a private of the 10th, (no Co. shown), Ga. Cav. surrendered May 10, 1865 at Tallahassee, Fla., and was paroled at Thomasville, Ga., May 20, 1865. Benj. The records show that P. S. Heeth, 2 Sergt., Company F, 20 Regt. Ga. Cav., C.S.A., enlisted July 11, 1863 and muster roll for Sept. and Oct., 1864, last on file, shows him wounded at Newmarket Aug. 16, 1864; furloughed from hospital, at home in Ga. The above named Company subsequently became a part of the 10 Regt. Ga. Cav., C.S.A., but the name has not been found on the rolls of said organization. No record of capture or parole. Robert C.
