Weeks do hereby make application to the State board of pensions for a pension to be granted to me under the act of 1907, Chapter 5600 of Laws of Florida on the following grounds: I am the widow of Ezekiel Weeks Pension # 2296 deceased, who departed this life on the 23rd day of April 1909 A. D. in the county of Hillsborough in the State of Florida. I was married to him on the 27th day of Dec. A. D. 1865 in the county of Bradford in the State of Florida. I have not remarried since the death of my husband Ezekiel Weeks. My husband, the said Ezekiel Weeks, enlisted and served in the military service in the Confederate State during the war between the States of the United States, and he did not desert the Confederate or State service. I have been a resident of the State of Florida for ten consecutive years prior to the date of this application and that I am not receiving a pension from any other state. {signed] Laura G. D. One Thousand Nine Hundred and nine, personally appeared before me, a Notary Public in and for the County and State aforesaid Laura G. Weeks a resident of Sydney Fla. County of Hillsborough State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of Chapter 885, Laws of Florida, approved June 7, 1909. That she is the widow of Ezekiel Weeks who was enlisted under the name of Ezekiel Weeks on the 20th day of May 1861, in Company "F" 4th Florida Regiment of the State of Florida and who was captured at Resaca GA. May 1864 on account of sent to Rock Island prison from which he was discharged March 27, 1860 by exchange. Suffering with Rheumatism which he suffered with during his life. He died April 23, 1909 for proof of service see proof made application # 2296. That she was lawfully married to the said Ezekiel Weeks under the name of Laura G. Simmons in the County of Bradford, State of Florida on the 26th day of January 1866 and that she was not divorced from him and that she has not remarried since his death, which occurred on the 23rd day of April, 1909, in the County of Hillsborough, State of Florida. That she is a resident of Hillsborough County, Florida, and has continuously resided in the State of Florida since the 1st day of Feb. 1861. That she does not own property, including real estate, personal property, mortgages or other collateral securities, stocks or bonds in this or any other State to exceed in value over the sum of Five Thousand Dollars.
