One Thousand Nine Hundred and fifteen personally appeared before me, a Clerk Circuit Court in and for the county and State aforesaid, James Aycock who, being duly sworn according to law, declares that he is 72 years of age, having been born on the 13th day of March, 1843, in the county of Randolph, in the State of Georgia. That he is a bona fide citizen of the county of Holmes, State of Florida. That he has resided in the State of Florida continuously since the 10 day of September 1906. That he is the identical person who enlisted at Macon, Ga., under the name of James Aycock, on the 20 day of May 1864, in Company E 3rd Ga. Regiment…………..of the State of Georgie (sic) in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate States Army and who was honorably discharged at Macon, in the State of Ga., on the 10 day of [October] April, 1865, on account of after Lee’s surrender on account of being wounded. The deponents further say that they have carefully examined the said applicant’s physical condition and find: (Here state nature, character and extent of wounds, disease or disability. Please avoid technical terms.) He was struck by a bullet about 3 inches above and to the right of left nipple making flesh wound and flesh wound to the scalp. Also struck in right leg by bullet going through flesh part of leg, all done while fighting in Civil war. Also has Chronic Rheumatism. This deponent further says that the said James Aycock is [permanently] partially disabled by reason of such Disabilities from earning a livelihood for himself by manual labor. W. D. E. 3d Georgia Reserves, C.S.A., enlisted April 16, 1864. On the latest roll on file, dated October 31, 1864, he was reported present. The records also show that he was one of a number of prisoners captured during April, 1864, and confined at Macon, Georgia. No further record of him has been found. H. J.
