D. One Thousand Nine Hundred and nine, personally appeared before me a Clerk of Circuit Court in and for the County and State aforesaid Mrs. Carrie E. Bellot a resident of LaFayette County, of Fletcher, in the State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of Chapter 5885, Laws of Florida, approved June 7, 1909. That she is the widow of David H. Bellot who was enlisted under the name of David H. Bellot, on the ____ day of ___, 1861, in Company H First Regiment of the State of South Carolina, Volunteers and who was honorably discharged at Appomattox Courthouse, VA Apr. 9, 1865, on account of surrender of Lee, and end of war. (Here give complete statement of other service, if any.) That he also served as Sergeant of Co. H First Reg. So. Ca. Vol. commanded by Col. Maxey Gregg. Afterwards known as "Gregg's Brigade." That she was lawfully married to the said David H. Bellot under the name of Carrie E. Harris in the County of Madison, State of SC. on the 22nd day of December, 1869, and that she was not divorced from him and that she has not remarried since his death, which occurred on the 26th day of August, 1899, in the County of LaFayette State of Florida. That she is a resident of LaFayette County, Florida, and has continuously resided in the State of Florida since the ____ day of January A. D. 1883. That she does not own property, including real estate, personal property, mortgages or other collateral securities, stocks or bonds in this or any other State to exceed in value the sum of Five Thousand Dollars. That the following is a true and correct statement of all property owned by me in this or any other State: Real estate, located near Fayetteville in LaFayette Co., and undivided 1/6 int. in 1853 acres - val. of my lot . . . . . . . . . $ 200.00 Cattle, horses and other live stock . . . . . . . . . . . . . . . . $ 100.00 Total . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . $ 300.00 That I have never heretofore applied for a pension from the State of Florida. McAlpin, DC (FORM C) Affidavit to be Made by a Commissioned Officer STATE OF SOUTH CAROLINA} County of Richland} Before me, a Clerk Circuit Court in and for the County and State aforesaid, personally came A. C. Haskell who being duly sworn according to law, declares that he was a commissioned officer with the rank of Lieutenant in Company I, First Regiment of the State of South Carolina during the War between the States of the United States; that he was enlisted at Abbeville in the State of South Caroline on the 3rd day of January, 1861, and that he was commissioned in the company "I" and regiment on the ____ day of August, 1861 and was commissioned as Captain and Adjutant General in November 1861 and as Colonel of 7th So. Ca. Cavalry Army ? in March 1864 and that he was HONORABLY DISCHARGED at Appomattox on the 9th day of April, 1865, by reason of the surrender of Army. That he personally knew David H. Bellot, whose widow is an applicant for a pension under the Laws of Florida; that he knows that the said David H. Bellot enlisted in Company H First Regiment of the State of S. C.; that he served faithfully until HONORABLY DISCHARGED on the 9th day of April 1865, on account of Lee's surrender and close of war at Appomattox State of Virginia, and that the said David H. Bellot did not desert the service of the Confederate States nor did he take the oath of allegiance to the United States before the close of the war. A. C. Haskell Private Co. D, Lieut. Co. I Sworn and subscribed before me, this 10 day of December, 1909; and I hereby certify that the above declaration was fully made known to the affiant before swearing, and that I have no interest, direct or indirect, in the pension claim sought to be established by this declaration. J. F. Watkin Clerk Circuit Court (FORM D) STATE OF SOUTH CAROLINA} County of Richland} Before me, a Clerk of Circuit Court (Richland Co.) in and for the State and County aforesaid, personally came H. C. Heise of City of Columbia County of Richland State of South Carolina, who, being duly sworn according to law, declares that he is the identical person who enlisted at Columbia in the State of South Carolina on the 8th day of August 1861 in Company "C" of the 1st Regt. So. Ca. Vol. Regiment of the State of South Carolina, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that David H. Bellot enlisted in Company "H" 1st Regiment of the State of So. Ca. in the State of South Carolina on the ? of summer of 1861; that he served faithfully in the said organization until the 9th day of April 1865; when he was HONORABLY DISCHARGED on account of the surrender of the Army of N. Va. at Appomattox in the State of Virginia, and did not desert the Confederate State service nor take the oath of allegiance to the United States before the close of the War. I know personally of the above stated service, because I served with David Bellot of Co. "H" in Company C of same Regiment 1st S. C. Vol. of the State of So. Ca., and was HONORABLY DISCHARGED at Appomattox in the State of Virginia on the 9th day of April 1865, by reason of the surrender of Army and I did not desert the Confederate States nor take the oath of allegiance to the United States before the close of the war.
