paternal grandparents, John, Jr. and Joan Lewis of Lake Wales; patrnal  great-
grandparents, Jack and Shirley McGhee-Babson Park; paternal great-grandmother, 
Myrtle Lewis of Bartow.

Additional Comments:
The child was a member of the First Assembly of God.
