J. Hagin, age 64 years, who being by me duly sworn, declares he is the identical person who enlisted on or about the 26th day of October, 1861 in Captain William Clark's Company, county of Alachua, in the State of Florida, and that while in actual service in said company, 1st Fla. Cavalry Regiment, of the State of Florida, and in the line of duty As such soldier, at "Ressacca-also near Mississippi Ridge and also at Chicamauga and skirmish near Rome, GA, State of all in Tennessee except the last on or about April 1862 - July 1862 - May 1863 - Nov. 1863" (He cannot remember the dates of the months) I was first wounded it was supposed with a cannon (conical?) ball which entered the back on right side between 7th and 8th rib about 1 inch from spine-the ball going through the body and lodging just under the skin just below and left of breast bone. It affects me whenever I take cold which I am subject to. It entirely disables me from working at such times and always troubles me more or less. The 2nd wound was caused by splinter knocked off a tree by a shell. The splinter entered the right buttock and lacerating it and the hip to such an extent that wound was sufficiently severe enough to lay me up in hospital for two months. It does not or very rarely causes me any inconvenience now. The 3rd injury resulted from a tree top falling on my head and fracturing my skull on a line about 6 or 7 long the fracture beginning about an inch & 1/2 back of left ear and extending upward across the arch of the head and through the place in the hair called commonly the crown to a spot about an inch & 1/2 the right side of this spot (see drawing in margin). This wound rendered me entirely unconscious for three days. This was the worse injury I received and it was a very severe wound very nearly resulting in my death. It has injured me ever since it was received impairing my mind & memory. At times I become giddy & dizzy and for a minute forget where I am and what I am doing or what I am talking about. This has also injured my hearing in right ear. Sometimes I suffer pain in head and neck especially if I have been exposed to the sun. The fourth wound was caused by a bursting shell-2 pieces striking my right hip around the top of the hip bone and a 4th piece striking me in my right shoulder. This wound laid me up for 3 months and ultimately caused me to be relieved from all further field duty. I do not suffer from this wound now that I know of. My general health though was ruined by the hard service in the Confederate Army. I never was strong and the War made me a ailing man ever since." And as a direct result of said injuries, thus received in line of duty during the war, I am now unable to gain a livelihood by manual labor; that I have continuously since January 1st, 1880, been a citizen of Florida; that neither I, nor my wife, nor both combined, own real and personal property to the value of $600 in this or any other State and have not purposely disposed of our property for the purpose of availing ourselves of the provisions of the pension laws of Florida; that I am not otherwise enabled or in a position to earn, and have no income from any source sufficient for, a livelihood, and that I have no children or other relation able so to do, and whose legal duty it is to support me, and that I never deserted the Confederate service, and that I receive a pension from no other source.
