That he is a bona fide citizen of the county of Levy, State of Florida. That he has resided in the State of Florida continuously since the 4th day of January, 1868. That he is the identical person who enlisted at Laurens Courthouse, under the name of Lewis Abercrombie, on or about the 15th day of February 1862, in Company A, Regiment 6th S.C. Cal of the State of South Carolina in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate States service, and who was Honorably Discharged at Appomattox C.H. Virginia, in the State of Virginia, on the 9th day of April, 1865, on account of the end of the war.
