Powers Company of 10 Fla Regiment from the State of Florida, and served in said Company; that while so serving, in the last named Company and in the legitimate discharge of duty he was on the about the 10 day of Sept., 1864, at Petersburg Va wounded by (Here state precise manner in which received) a shell in the foot and that as a direct result of said wound, he is now unable to perform manual labor; that he is not a pensioner of any other State, or of the United States; that he has resided continuously the last 15 years in The State of Florida, and respectfully makes this claim for the pension authorized by the Act approved February 16th, 1885, and amended June 7th, 1887. Witherspoon County Judge I Swear that I have carefully examined the above applicant, Samuel Agnes who is to me personally well known to be the man above represented and find (Here state location and extent of wound, and why it “renders unfit to perform manual labor.”) that he was wounded in the foot, by which he lost a part of the bones which has left him permanetly (sic) lame and rendered unfit to do a full days work resulting direct in the loss of……… Jos. Witherspoon I Swear that I have carefully examined the above applicant, ……………………, who is to me personally well known to be the man above represented and find (Here state location and extent of wound, and why it “renders unfit to perform manual labor.”) that the ball entered his left foot at the first metatarsal bone not far from the (illegible) toe and lodged in the external (illegible) bone. The effect of the wound has been to destroy all function in in (sic) the metatarsal and (illegible) bones, resulting directly in the loss of the use of the foot. I. A. Witherspoon County Judge Page 004 State of Florida} County of Madison} We, the Board of County Commissioners of said county, do certify that we have carefully investigated the statements made in the within affidavits, and are satisfied by evidence produced before us, that the within described applicant is the veritable Samuel Agnes who enlisted and served in Captain Powers Company 10th Regiment, from the State of Florida; that he is, by reason of wounds received in line of duty, while so serving, rendered unfit to perform manual labor; that he is a citizen of this county, has resided in this State continuously for the last 15 years, and that he is not receiving a pension from any other State, or from the United States.
