George Allen Pendarvis, b. 2 Aug 1838 in Duval Co. FL and d. 27 Feb 1921 in Manatee County, Florida and buried in Palmetto Cemetery, Manatee Co., Florida, m. Mary Ann Wingate, b. 26 Dec 1835 in Fernandino Beach, Forida died 4 Jul 1897. She is the daughter of William and Margaret Wingate of Nassau County, Florida. Application No. A06403 STATE OF FLORIDA,} Manatee County} On the 12th day of May, 1904, personally appeared before me, Clerk of the Circuit Court in and for said county and State, George Allen Pendarvis, who being by me duly sworn, declares he is the identical who enlisted on or about the 16th day of May, 1862, in Captain Robert Harrison's Company, county of Nassau, in the State of Florida, and that while in actual service in said company, 2nd Cavalry Regiment, of the State of Florida and in line of duty as such soldier, at "was in prison at close of war and not formally discharged, I was held in prison sometime after close of war until about July 25, 1865. I was captured by the U.S. Troops at Baldwin, Florida on or about July 26, 1864, and I was so held till close of war, when at close of war I was discharged from there and came direct home to Callahan", State of Florida. (Here state fully and clearly all the facts, showing the injury, its character and especially the extent of the injury and disability resulting therefrom.) "I was sick of measles and from a relapse of which I have never fully recovered having contracted muscular rheumatism, which so disables me at times I cannot move about, and as I grow older the trouble becomes much more troublesome. I was born August 2nd A.D. 1838 and lived 66 years." And that as a direct result of said injuries, thus received in line of duty during the war, I am now unable to gain a livelihood by manual labor; and that I have continuously since January 1st, 1885, been a citizen of Florida; and that neither I nor my wife, nor both combined, own real or personal property to the value of $800, in this or any other state, and that I have not personally disposed of our property for the purpose of availing ourselves of the provisions of the pension laws of Florida; and that I am not otherwise enabled, or in a position to earn, and have no income from any source sufficient for a livelihood by manual labor, and that I never deserted the Confederate service, and that I receive a pension from no other source. Stancliff Deputy Clerk I do solemnly swear that I was born on the second (2) day of August 1838 in Duval county, State of Florida, and that I am now past the age of sixty-five (65) years; that I was a bona-fide citizen of this State on January A. D. 1885, and have continuously resided in this State since that date; that I do not own property, real or person, or real and personal, to the value of eight hundred dollars ($800), and that I am by reason of age incapable of providing a living by manual labor for myself. Pendarvis, who is personally known to us to be the person under the laws of Florida, and find that in the consequences of chronic rheumatism very probably the result of exposure during the war of 1861-1865, and in consequence of physical disability as a result of old age - 66 years, is at least, at times unable to earn a livelihood for himself by manual labor M. C. Higginbotham, a Justice of the Peace, in and for the fifth district of said county personally came S. D. Jones who being duly sworn says that he served with George Allen Pendarvis in Capt. Robert Harrison's Company-K afterwards Capt. Jessie Jones-2nd Florida Regiment Cavalry - Confederate Army - and that he remembers that the said George Allen Pendarvis had measles while in said service, and that said George Allen Pendarvis was captured by the Federal troops at or near Baldwin, Fla. in the summer of 1864 - and that the said George Allen Pendarvis returned to Callahan, Fla. after the close of the war and from deponents agnitum with the said Pendarvis he believes is suffering from the diseases complained of in his application hereto attached - deponent further says that the George Allen Pendarvis never deserted the service of the Confederate Army. D. One Thousand Nine Hundred and Nine personally appeared before me, a Notary Public in and for the county and State aforesaid, George A. Pendarvis, who, being duly sworn according to law, declares that he is 71 years of age, having been born on the 2nd day of August, 1838 in the county of Duval in the State of Florida. That he is a bona fide citizen of the county of Manatee, State of Florida. That he has resided in the State of Florida continuously since the ALL MY LIFE. That he is the identical person who enlisted at Jacksonville under the name of George A. Pendarvis on the 16 day of May 1862, in Company K, Regiment 2nd Fla Cavalry of the state of Florida in the service of the Confederate States Army and was HONORABLY DISCHARGED at Hilton Head in the State of South Carolina at close of war 1865. (Here state fully any other military service performed by the applicant.) I enlisted in J. J. Daniel's Co. at commencement of war served for one year but was discharged for disability before the year was out. Came home got well and joined the army again as above mentioned. (Here give date and place of capture, imprisonment, exchange or parole.) Paroled at Hilton Head at close of war. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States Army in the year 1865, and did not desert the service of the Confederate States Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of war.) That I was in the Army a prisoner at Hilton Head, S.C. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. D. who being duly sworn, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows George A. Pendarvis the applicant named in the foregoing application for a pension. This deponent further states that he has carefully examined the said applicant's said conditions and find: (Here state nature, character, and extent of wounds, disease, or disability. Please avoid technical terms.) He is old and infirm unable to make a living, is alone and no income of any kind and is a sufferer from rheumatism contracted in the army. This deponent further says that the said George A. Pendarvis is permanently____ disabled by reason of such old age and total disability from earning a livelihood for himself by manual labor.
