M. Blair, who enlisted March, 1862, in Captain James Dunklin’s Company, ………….Regiment of the State of Alabama, and who while in actual service and in the line of duty as such…………., was on the….day of…………., 186….., at…………………..in the State of ………………… (Here state fully and clearly all the facts showing date and cause of death.) and who died July 12th 1902 and who at the date of his death was a pensioner of the State of Florida, enrolled as pensioner #831, and for proof of service of service of (sic) said James M. D. 1890; that I do not own property, real and personal, to the value of $800 in this or any other State, and am not otherwise enabled, or in a position to earn a livelihood, and have no income from any source sufficient for a livelihood; and that I have not purposely disposed of my property for the purpose of availing myself of the provisions of this act. The children of parents who are unable to support themselves, shall be required to make provisions for their support. 2078. On information filed before the Justice of the Peace of the proper district by any person whomsoever, stating that certain persons have made no adequate provisions for their father and mother, or either of them, the Justice shall cause a summons to be issued to said parties, and evidence to be taken as to the truth of the facts stated in the information, and if the same shall be found true, after a fair trial in which the defendants shall have the right to be heard by counsel, the Justice shall issue an order making an assessment on the said children for such amount as shall be necessary for the support of their parents. 2079. Blair, aged 73 years, by occupation a Farmer who, being duly sworn according to law, declares that he is the identical Person who was, on the….day of March, 1862, enrolled in Captain James Dunklin Company C of Butler County, State of Alabama, and that while serving as a Private in C Company 33rd Regiment, Hardee’s Brigade, and in the line of his duty at 12 Miles South of Corinth in the State of Miss., on or about the May day of……., 1862, (Here state name or nature of disease, or location of the wound or injury. If disabled by disease, state fully the causes; if by wound or injury, the precise manner in which received, and especially the extent of the disability resulting therefrom.) I was pushed of a Bridge on a dark rainey night while our Company were being rushed across to form a line of battle, I fell about twenty feet into the mud and water, resulting in a cripled (sic) shoulder and a diseased eye and that as the result of said injuries I am now unable to earn a livelihood; that I am a citizen of said County and State, and have been a bona fide citizen of the State of Florida continuously since January 1st, 1880; that neither I nor my wife owns property to the value of fifteen hundred dollars in this or any other State, and that I am not “otherwise enabled or in a position to earn a livelihood;” that I am not receiving a pension from the United States, or any other State; that I never deserted the Confederate service, and make this declaration for the purpose of being placed upon the invalid pension roll of the State of Florida, as provided by Act of the Legislature, approved June 8, 1889, as amended June 2, 1893 and June 15, 1897. Blair who is to us well known as the applicant for pension above described, and find that (Here describe fully and clearly the wound or disease, and especially the extent of the resulting disability.) He is suffering from Chronic Piles, which often bleed considerably. He also has (illegible) of right eye almost covering the pupil, which causes considerable pain. He complains of constant pain in this eye & I can find no other cause for it. He is very feeble & almost wholy (sic) unable to work on account of piles. He suffers from pain in right shoulder & has for years, caused as he says from falling from a bridge during the war………………. Resulting directly in the loss of……………………………………………………………. W. V. Those parts would be very difficult to fill out here but can easily be copied from Mr. Blair’s papers on file in the Controller’s office. Hadnot a Notary Public duly commissioned qualified and sworn according to law in and for the Parish of Grant, State of Louisiana, personally came and appeared Jasper Blair, a citizen of the Parish of Grant, State of Louisiana, whose post office is Colfax, La., a citizen well known to me to be a truthful and reliable who after being by me notary duly sworn according to law deposes and says that he is well acquainted with J. M. Blair now a resident of Marion County State of Florida that he the said Jasper Blair knows of his personal knowledge that said J. M. Blair enlisted in the Confederate army during the year 1862 at Greenville Alabama, 1st March 1862 in the 33 Regiment under Captain James Dunklin. That said J. M. Blair served during the Page 013 Entire war of the Rebellion or civil war. That the said J. M. Blair was made a prisoner and carried to Ship Island and confined there until the end of the war. Deponent further avers that he has no interest in the pension claim of said J. M. Blair. And further avers that he and the said J. M.
