That he is the identical person who enlisted at Petersburg Va, under the name of Dennis H. Ledbetter, on the 19 day of Feb, 1864, in Company D, Regiment Phillips Legion of the State of Ga in the service of the Confederate Army (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) and who was HONORABLY DISCHARGED at Warsaw, in the State of Virginia, on the 10 day of May, 1865, on account of Surrender (Here state fully any other military service performed by applicant.)…………………………………………………………………………… (Here give date and place of capture, imprisonment, exchange or parole.) Wynn, of Rome County of Floyd, State of Georgia who, being duly sworn according to law, declares that he is the identical person who enlisted at Big Shanty or Kennasaw in the State of Georgia on the …..day of April 1861, in Company D, Phillips Legeon (Sic) Infty Regiment of the State of Georgia, which organization was regularly mustered into the service of the Page 007 Confederate States of America; that he knows of his own personal knowledge that Dennis Hankins (sic) Ledbetter enlisted in Company D, Phillips Legeon (Sic) Infty Regiment of the State of Georgia, don’t remember just when or where, but after said Regt. reached Va., the said Ledbetter joined our Company on the 19 day of Feb., 1864; that he served faithfully in the said organization Affiant does not remember when the said Ledbetter was discharged or surrendered. The said Ledbetter made a good soldier, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War.
