One Thousand Nine Hundred and 17 personally appeared before me, a Notary Public in and for the county and State aforesaid, John D. Small who, being duly sworn according to law, declares that he is 76 years of age, having been born on the 27 day of May, 1841, in the county of Lancaster, in the State of South Carolina. That he is a bona fide citizen of the county of Marion, State of Florida. That he has resided in the State of Florida continuously since the 6 day of June, 1910. That he is the identical person who enlisted at Pleasant Plains, under the name of John D. Small, on the 28th day of Aug., 1861, in Company E, Regiment 12th of the State of South Carolina in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate States of America and who was honorably [discharged] furloughed at Charlottsville, in the State of Virginia, on the 20th day of June, 1864, on account of Gun Shot from Enemy causing (Here state fully any other military service performed by the applicant.) loss of right arm. That I was furloughed from the hospital at Charlottsville Virginia on the 20 day of June 1864, reached my home on July 6th, 64 and did not do any active service after that time. That my furlough was extended from time to time by my officers until the close of the War. (Here give date and place of capture, imprisonment, exchange or parole.) That I remained at home after July 6th, 64, by authority of said furlough. That I served faithfully until honorably [discharged] furloughed from the service of the Confederate States in the year 1864, and did not desert the service of the Confederate States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. Sims of Lancaster or Heath Springs, County of Lancaster, State of South Carolina who, being duly sworn according to law, declares that he is the identical personal who enlisted at Pleasant Plains in the State of South Carolina on the 28 day of Page 007 Aug 1861 in Company E 12th Regiment of the State of South Carolina, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that John D. Small enlisted in Company E. 12th Regiment of the State of South Carolina Pleasant Plains in the State of South Carolina on the 28 day of Aug. 1861; that he served faithfully in the said organization until the 20th day of June, 1864, when he was honorably discharged on account of loss of right arm by gunshot at Spotsylvania C. H. in the State of Virginia, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. (Here state whether the soldier was transferred to another organization, or not, and if captured or imprisoned, state when and where.) Not Transferred. I know personally of the above stated service, because I served with John D. Small in Company E Regiment 12th of the State of S. C. and was honorably discharged at Appomattox C. H., in the State of Virginia, on the 9 day of April, 1865, by reason of Surrender of Army and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. Small for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000, and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant.
