D. One Thousand Nine Hundred and Sixteen personally appeared before me, a notary public in and for the county and State aforesaid, John W. Adkison who, being duly sworn according to law, declares that he is 72 years of age, having been born on the 16 day of Jan., 1844, in the county of Coffee, in the State of Ala. That he is a bona fide citizen of the county of Santa Rosa or Okaloosa, State of Florida. That he has resided in the State of Florida continuously since the 6 day of Feby, 1906. That he is the identical person who enlisted at Andalusia, Ala., under the name of John W. Adkison, on the 2 day of April 1862, in Company I, Regiment 40th of the State of Ala. in the service of the Confederate States and transferred to 15th Confederate Regiment Cav. May 15, 1862 at Mobile, Ala. and who was HONORABLY DISCHARGED at Big Black near Vicksburg, in the State of Miss., on the 7 day of May, 1865, on account of service no longer being required. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States in the year 1865, and did not desert the service of the said States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was in the State of Miss. when discharged and walked home, which was in Ala. near where Falco P. O. now is. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. That the following is a true and correct statement of all the property owned by me or by my wife, jointly and separately in this or any other State: Real estate, located at Blackman . . . . . . . . . . . . . . . . . . $1,000 Cattle, horses and other livestock . . . . . . . . . . . . . . . . . $ 500 Total. . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . $1,500 That I have heretofore applied for a pension from the State of Florida and refer to Claim No. ____ for the proof contained therein. Adkison, who is an applicant for a pension under the laws of Florida, and that from our own personal knowledge, and from the best information available, we believe that the applicant does not own property (including the property of his wife) to exceed in value the sum of $5,000, and that the statements made by him relative to the value of his property are true and correct. P. ******************************************************************************* FORM C Physicians' Affidavit State of Florida,{ County of Okaloosa{ Before me personally came M. Baggett, and W. A. Hooten, who being duly sworn each for himself, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows John W. Adkison the applicant named in the foregoing application for a pension. The deponents further say that they have carefully examined the said applicant's physical condition and find: (Here state nature, character and extend of wounds, disease or disability. Please avoid technical terms.) John W. Adkison to be wholy incable of earning a livelihood by reason of physical disabilities, caused by infirmities of age and gunshot wounds inflicted on both breast and on right thigh. This deponent further says that the said John W. Adkison is permanently ____ disabled by reason of such as stated above from earning a livelihood for himself by manual labor. M. Baggett, Physician W. Elliott of Bradley County of Escambia State of Ala. who, being duly sworn according to law, declares that he is the identical person who enlisted at Pollard in the State of Ala. on the 2 day of April 1862 in Company I 15th Confederate Regiment of the State of Ala., which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that John W. Adkison enlisted in Company I 40th Conf. Regiment of the State of Ala. and transferred to 15th Reg. at Mobile in the State of Ala. on the 2 day of April 1862; transferred May 15, 1862 at Mobile, Ala; that he served faithfully in the said organization until the 7 day of May 1865; when he was HONORABLY DISCHARGED on account of service no longer required at Big Black near Vicksburg in the State of Miss., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. (Here state whether the soldier was transferred to another organization, or not, and if captured or imprisoned, state when and where.) Enlisted Apr. 2/62 at Andalusia, Ala. and transferred to Co. I 15th Confederate Cav. Ala. Reg. of Confederate States. I know personally of the above stated service, because I served with John W. Adkison in Company I Regiment 40th & 15th of the State of Ala., and was HONORABLY DISCHARGED at Big Black above Mobile, Ala. in the State of Miss., on the 7 day of May 1865, by reason of service no longer required and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. J. B. Adkison, of Florala, County of Covington, State of Alabama, who, being duly sworn according to law, declares that he is the identical person who enlisted at Camp ?, in the State of Alabama, on the 15 day of Feby 1863, in Company I 15th Conf. Cav., Regiment of the State of Ala., which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that John W. Adkison enlisted in Company I 40th Conf. Regiment of the State of Ala., at Mobile, in the State of Ala. on the 15 day of May, 1862; that he served faithfully in the said organization until the 7th day of May, 1865, when he was HONORABLY DISCHARGED on account of ? rolled, at Big Black, in the State of Miss., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. Adkison in Company I 40th Ala., Regiment & 15th Cav., of the State of Ala., and was HONORABLY DISCHARGED at Big Black, in the State of Miss., on the 7th day of May, 1865, by reason of ? rolled, and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. Adkison for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000, and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant. Adkison who is an applicant for a pension under the laws of Florida, claims to have been a member of Company I Regiment 40th Ala., Confederate States Army, and to have been transferred May 15, 1862 at Mobile, Ala. to 15th Confed. W. (not borne as Jno. W.) Adkison, private, Co. I, 40th Alabama Infantry, C. S. A., enlisted May 10, 1862, at Mobile, Ala. On the company roll dated Dec. 31, 1863 (last on file), he was reported present. No record has been found of his transfer to Co. I, 15th Confederate Cavalry, nor has any later record been found.
