W. Campbell, do hereby make application to the State Board of Pensions, for a pension to be granted me under the act of 1907, Chapter 5600 of the Laws of the State of Florida, upon the following grounds: I enlisted and served in the 2nd Mississippi Cavalry Co. "B" service of Confederate States during the war between the States of the United States, and that I did not desert the Confederate or State service; that I was honorably discharged or surrendered; surrendered (as seen below) that I was a bona fide citizen of this State for ten years prior to the date of this application and have been continuously since a citizen of the State of Florida, and that I (Here state fully the disability under which the applicant claims a pension, whether he lost in service a limb or limbs, eye or eyes, or whether he is permanently disabled by reason of wounds received in service or disease to gain a livelihood by manual labor, or whether he is over 60 years of age and is by reason of age, incapable of providing a living for himself.) I was captured at Selma, Alabama April 3rd 1865. Was held by the federate as a prisoner until about the 15th of April 1865 and was then paroled. Was wound near Atlanta, Ga. about the first of August 1864. I further represent to the State of Board of Pensions, that I am not receiving a pension from any other State. I find him 72 yrs. old unable to do manual labor and suffering from chronic rheumatism and it concomitant symptoms. This deponent further says that the said G. W. Campbell who, being duly sworn according to law, declares that he is 72 years of age, having been born on the 18th day of August, 1837, in the county of Russell, in the State of Alabama. That he is a bona fide citizen of the county of Santa Rosa, State of Florida. That he has resided in the State of Florida continuously since the ____ day of about the year, 1883. That he is the identical person who enlisted at Decatur, Miss., under the name of G. W. Campbell, on the ____ day of March 1862, in Company "B", Regiment 2d Cav. of the State of Mississippi in the service of the Confederate States Army and who was HONORABLY DISCHARGED at ____, in the State of ____, on the ____ day of ____, 186_, on account of ____. (Here give date and place of capture, imprisonment, exchange or parole.) Apr. 3rd 1865 at Selma, Ala. kept here in a stockade as a prisoner and after about ten or twelve was march to Montgomery and kept then till paroled. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States Army in the year 1865, and did not desert the service of the Confederate States Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was in Montgomery, Ala. - prisoner of war. Mays, House of Representatives Sir: Returning herewith the letter, received by your reference today, of Mr. G. W. Campbell, of Holt, Florida, who requests your aid in obtaining his military record as a member of Company B, 2d Mississippi Cavalry, and the record of his capture and parole as a member of Armstrong's Brigade, Jackson's Division, I have the honor to advise you as follows: The Confederate records show that G. W. Campbell enlisted March 8 or April 8 (both dates shown), 1862, at Decatur, Mississippi as a private of Company B, 2d Mississippi Cavalry, Confederate States Army, to serve three years. On the muster roll of that company for November and December, 1864, the last on file, he was reported as present. The Union records of prisoners of war show that this soldier was surrendered as a member of Armstrong's Brigade, at Citronelle, Alabama, May 4, 1865, and was paroled at Columbus, Mississippi, May 16, 1865. W.
