D. One Thousand Nine Hundred and 16 personally appeared before me, a Justice of Peace in and for the county and State aforesaid, D. C. Hamond who, being duly sworn according to law, declares that he is 72 years of age, having been born on the 28 day of May, 1844 in the county of Spalding, in the State of Georgia. That he is a bona fide citizen of the county Okaloosa, State of Florida. That he has resided in the State of Florida continuously since the 4 day of October, 1906. That he is the identical person who enlisted at Demopolis, Ala., under the name of D. C. Hamond, on the don't remember day of ____ 1863, in Company H, Regiment 37 Ala. of the State of Ala. in the service of the Confederate States and who was HONORABLY DISCHARGED at Dalton, Ga., in the State of Ga., on the ____ day of January, 1864, on account of epileptic fits. After I was discharged in January 1864 in April 1864 I reenlisted in Company I 6 Alabama Calvary. (Here give date and place of capture, imprisonment, exchange or parole.) Wounded on 9 day of March 1865. Surrender 1865. That I served faithfully until HONORABLY DISCHARGED from the service of the ____ in the year 186_, and did not desert the service of the ____ nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was wounded a few days before the surrender. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. That the following is a true and correct statement of all the property owned by me or by my wife, jointly and separately in this or any other State: Real estate, located at . . . . . . . . . . none Cattle, horses and other livestock. . . . . none Personal property . . . . . . . . . . . . . none Stocks. . . . . . . . . . . . . . . . . . . no Bonds. . . . . . . . . . . .. . . . . . . . no Mortgages, notes and other securities . . . no (Here state any wounds received, or loss of limbs and eyesight.) Hamond, who is an applicant for a pension under the laws of Florida, and that from our own personal knowledge, and from the best information available, we believe that the applicant does not own property (including the property of his wife) to exceed in value the sum of $5,000, and that the statements made by him relative to the value of his property are true and correct. Richbourg (FORM C) Physicians' Affidavit State of Florida,{ County of ____{ Before me personally came Dr. E. Porter Webb, and Dr. S. E. Stephens, who being duly sworn each for himself, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows ____ the applicant named in the foregoing application for a pension. The deponents further say that they have carefully examined the said applicant's physical condition and find: (Here state nature, character and extent of wounds, disease of disability. Please avoid technical terms.) He is wounded in the thigh of left leg and by reason of age is not capable of earning a living for himself and wife. This deponent further says that the said D. C. Hamond is 3/4 disabled by reason of such wounds and age from earning a livelihood for himself by manual labor. E. Porter Webb, M. D. S. E. Stephens, M. D. Richbourg (FORM E) Affidavit to be Made by Comrade State of Florida} County of Okaloosa} Before me, a Justice of the Peace in and for the State and County aforesaid, personally came T. S. Cabaniss of ____ County of Santa Rosa State of Florida, who, being duly sworn according to law, declares that he is the identical person who enlisted at Talladega in the State of Alabama on the 29 day of March 1862 in Company F of 31 Ala. Regiment of the State of ____ which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that H. C. Hamond enlisted in Company H 37 Regiment of the State of Ala. at Demopolis in the State of Ala. on the ____ day of Aug. 1863; that he served faithfully in the said organization until the ____ day of January 1864; when he was HONORABLY DISCHARGED on account of epileptic fits Jan. 1864 at Dalton in the State of Ga., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. (Here state whether the soldier was transferred to another organization , or not, and if captured or imprisoned, state when and where.) Soldier reenlisted in April 1864. Wounded on 9 of March 1865. Shot in left thigh. I know personally of the above stated service, because I served with D. C. Hamond in Company I Regiment 6 of the State of Ala., and was HONORABLY DISCHARGED at shout out, one month before surrender and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. Sworn and subscribed before me this 5 day of October, 1916, and I hereby certify that the above declaration was fully made known to the affiant before swearing, and that I have no interest, direct or indirect, in the pension claim sought to be established by this declaration. J. R. Richbourg (FORM F) Affidavit to be Made by Comrade State of Alabama} County of Escambia} Before me, a Notary Public, in and for the State and County aforesaid, personally came S. S. Thompson, of Brewton, Ala. Rt. 5, County of Escambia, State of ____, who, being duly sworn according to law, declares that he is the identical person who enlisted at Mobile, in the State of Alabama, on the ____ day of about Feb. 1862, in Company I 29th Regiment of the State of Alabama, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that H. C. Hammonds enlisted in Company H, 37th Reg. in the State of Alabama, at Demopolis, in the State of Alabama on the ____ day of August, 1863; that he served faithfully in the said organization until the ____ day of Jan., 1864, when he was HONORABLY DISCHARGED on account of epileptic fits, at Dalton, in the State of Georgia, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. (Here state whether the soldier was transferred to another organization, or not, an dif captured or imprisoned, state when and where.) Soldier was reinstated Pollard, Alabama, in April 1864, Company I, 6th Alabama Cavalry. I know personally of the above stated service, because I served with him in same Army, in Company I, 29th Regiment of the State of Alabama, and was HONORABLY DISHCARGED at Montgomery, in the State of Alabama, about May, 1865, by reason of close of war, and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. C. Hammonds for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000, and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant. A. was enlisted April 14, 1863. On muster roll dated Dec. 18, 1863 - last one file - presence or absence not stated but previous roll Oct 31, 63 shows him present. Union prisoner of war records show him as captured in Florida on March 25, 1865 and paroled at Camp Townsend May 6, 1865. H. T.
