D. One Thousand Nine Hundred and 16 personally appeared before me, a Charles S. Kelly in and for the county and State aforesaid, who, being duly sworn according to law, declares that he is 75 years of age, having been born on the have no record day of 18___, in the county of Henry, in the State of Ala. That he is a bona fide citizen of the county of Okaloosa, State of Florida. That he has resided in the State of Florida continuously since the day of twelve years, 18__. That he is the identical person who enlisted at Abbeville, under the name of Charles S. Kelly, on the day of don't remember, 186_, in Company A Home Guard, Regiment ____ of the State of ____ in the service of the in the service of State of Ala. and who was HONORABLY DISCHARGED AT never discharged, in the State of at home under parole, on the ___ day of ___, 186_, on account of being captured by the federals. (Here state fully any other military service performed by the applicant.) On account of being captured, I was transferred to the salt works at West Bay, Florida where I stayed and made salt until I was captured and was paroled at that time before the war ended. My best recollection is I was transferred to the salt works in the latter part of the year 1863 and worked there about a year. (Here give date and place of capture, imprisonment, exchange or parole.) Don't remember date of capture That I served faithfully until HONORABLY DISCHARGED from the service of the taken prisoner in the year 186_, and did not desert the service of the _____ nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was at home in Henry County, Ala. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. Porter Webb, M. D., who being duly sworn, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows Chas. S. Kelly the applicant named in the foregoing application for a pension. This deponent further says that he has carefully examined the said applicant's physical condition and finds: (Here state nature, character and extent of wounds, disease or disability. Please avoid technical terms.) kidney affections, rupture, and head trouble, and old age This deponent further says that the said Chas. S. (Form E) Affidavit to be Made by Comrade. State of Alabama} County of Henry} Before me, a _______ in and for the State and County aforesaid, personally came J. W. McClenny of Headland, County of Henry State of Alabama, who, being duly sworn according to law, declares that he is the identical person who enlisted at Abbeville in the State of Alabama first of the year 1863 in Company of S. J. ? Home Guards Regiment of the State of Major Culver Community, which organization was regularly mustered into the service of the Confederate States of Americas; that he knows of his own personal knowledge that Charles S. Kelly enlisted in said Company ____ Regiment of the State of Alabama at Abbeville in the State of Alabama on the first of the year 1863; that he served faithfully in the said organization until the ___ day of ___ 186_, when he was HONORABLY DISCHARGED on account of _______ at ____ in the State of _____, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. I know personally of the above stated service, because I served with the said Charles S. Kelly in Company S. J. ? Company Home Guards of the State of Alabama, and was discharged at close of the war, in the State of Alabama, on the ____ day of ____186_, by reason of ____ and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the war. Sworn and subscribed before me this 20 day of Oct., 1916, and I hereby certify that the above declaration was fully made known to the affiant before swearing, and that I have no interest direct or indirect, in the pension claim sought to be established by this declaration. State of Alabama} County of Henry} Before me, a ____ in and for the State and County aforesaid, personally came A. B. Kelly, of Dothan, County of Houston, State of Alabama, who, being duly sworn according to law, declares that he is the identical person who enlisted at Abbeville, in the State of Alabama, on the ____ day of Nov. 1863, in Company A Home Guards Regiment of the State of Alabama, which organization was regularly mustered into the service of the Confederate States of America; that he knows of his own personal knowledge that Charles S. Kelly enlisted in Company A Home Guards Regiment of the State of Ala., at Abbeville, in the State of Ala. on the ____ day of Nov., 1863; that he served faithfully in the said organization until paroled 1864, when he was HONORABLY DISCHARGED on account was captured at West Bay and paroled in the State of Fla., and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the war. (Here state whether the soldier was transferred to another organization, or not, and if captured or imprisoned, state when and where.) Charles S. Kelly being ruptured was in 1863 transferred to the salt works at West Bay, Fla. & served there until captured by the federals and paroled. I know personally of the above stated service, because I served with Charles S. Kelly, in Company A Home Guards of the State of Ala., and was HONORABLY DISCHARGED at Home, Henry Co., in the State of Ala., 1865, by reason of close of the war, and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the war. Soloman, Notary Public Report of County Commissioners We, the undersigned, County Commissioners in and for the County of Okaloosa Florida, do hereby report that at a meeting of the Board of County Commissioners held this day, the foregoing application of Charles S. Kelley for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000, and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant.
