D. One Thousand Nine Hundred and 15 personally appeared before me, a notary public in and for the county and State aforesaid, Noah D. Peacock who, being duly sworn according to law, declares that he is 77 years of age, having been born on the ____ day of September, 1838, in the county of Dallas, in the State of Alabama. That he is a bona fide citizen of the county of Santa Rosa - alias - Okaloosa, State of Florida. That he has resided in the State of Florida continuously since the ____ day of September, 1905. That he is the identical person who enlisted at Newton, under the name of Noah D. Peacock on the 1st day of September, 1862, in Company F, Regiment 15th Reg. of the State of Alabama in the service of the Confederate States Army and who was furloughed at Richmond, Va. Mar 9, 1865 a/c of wound in leg at Barnes Cross Road, in the State of Alabama, on the ____ day of April, 1865, on account of war closed. (Here give date and place of capture, imprisonment, exchange or parole.) November 1863, siege of Knoxville, Tenn. Imprisoned at Camp Chase Ohio, exchanged at Richmond, Va. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate Army in the year 1865, and did not desert the service of the Confederate States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was at home on furlough. Reported back to my same Capt. Bean at Barnes Cross Roads in Alabama and was mustered out by him. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. Armstrong Notary Public (FORM C) Physicians' Affidavit State of Florida, County of Okaloosa} Before me personally came Gus. O. Spears, and Geo. W. McLarty, who being duly sworn each for himself, deposes and says, that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows Noah Dallas Peacock the applicant named in the foregoing application for a pension. The deponents further say that they have carefully examined the said applicant's physical condition and find: (Here state nature, character and extent of wounds, disease or disability. Please avoid technical terms.) His left leg between ankle and knee being shot while on duty in the Confederate war was wounded in the fall of 1863 of same war. This deponent further says that the said Noah Dallas Peacock is permanently physically disabled by reason of such left leg being shot from earning a livelihood for himself by manual labor. Gus O. Morris Notary Public ******************************************************************************* Affidavit to be Made by Comrade State of Florida} County of Escambia} Before me, a Clerk Court of Record, in and for the State and County aforesaid, personally came H. B. Fredrick of Pensacola R. F. D. No. 2 County of Escambia, State of Florida, who, being duly sworn according to law, declares that he is the identical person who enlisted at Pollard in the State of Alabama on the 2nd day of June 1861, in Company A 19th Louisiana Regiment of the State of Louisiana, which organization was regularly mustered into the service of the confederate State of America; that he knows of his own personal knowledge that Richard Morgan enlisted in Company G 19th Regiment of the State of Louisiana at Dalton, in the State of Georgia, on the ____ day of ____, 1863; that he served faithfully in the said organization until he was wounded; when he was HONORABLY DISCHARGED on account of wounds being struck in 5 places, at Atlanta in the State of Georgia, and did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the War. I know personally of the above stated service, because I served with him in Company G Regiment 19 of the State of La. (Gibson's Brigade), and was HONORABLY DISCHARGED at May 1865, in the State of Alabama. He being at home on furlough on account of wounds and I did not desert the Confederate States service nor take the oath of allegiance to the United States before the close of the war. His X mark: H. Peacock for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000, and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant. Windbaur Justice of the Peace Report of the Board of Examiners of Crenshaw County. Applicant came in person before the Board of Examiners and was sworn and examined and to separate questions as to each allegation in application answered that it was true as therein stated, and that the appraisement of each piece of property as set out in schedule was a fair and true valuation of the same. The board is of the opinion, that the statement as to the value of the property is substantially true, and that the total valuation of the property does not exceed four hundred dollars. On physical examination, we find that his wounds and age is not sufficient to render him unfit for manual labor and that he is able to make support. We find the applicant on physical examination at this time feeble, pale, and suffering from his wounds. I think he should have a pension as his age now with his wounds makes him too feeble to perform much work. ******************************************************************************* The State of Alabama,} Crenshaw County} Before me, B.
