D. One Thousand Nine Hundred and 17, personally appeared before me, a Notary Public in and for the County and State aforesaid Mary Jane Paul a resident of Laurel Hill County of Okaloosa State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of Chapter 7259, Laws of Florida, approved June 7, 1917. That she is the lawful widow J. W. Paul, now deceased who enlisted under the name of J. W. Paul on the ____ day of May, 1863, in Company F 11th Florida Regiment of the State of Florida and who was honorably discharged at close of war Apr. 9th, 1865, on account of war closing. That he also served for proof of husband's service refer to #5618. That was lawfully married to the said J. W. Paul under the name of Mary Jane Faircloth in the County of Holmes State of Florida on the 29th day of November, 1867, and she was not divorced from him before nor has she remarried since his death, which occurred on the 8th day of November, 1917, in Chattahoochee the County of ____, State of Florida except as hereinafter stated. That she is a resident of Okaloosa County, Florida, and has continuously resided in the State of Florida since the 15 day of January 1851 (I was born in State of Florida and resided in this state all my life.) That she was heretofore granted a pension from the State of Florida under Certificate No. 5618 for J. W. Paul. That she is not a pensioner of any other State. That her Postoffice address is Laurel Hill, County of Okaloosa, State of Florida. Her X mark: Mary Jane Paul Attest: (1) T. R. Paul and Mary Jane Faircloth which occurred at Jacob Faircloth on the 29 day of November, A. Vanlandingham Notary Public, State of Florida at Large Report of County Commissioners We, the undersigned, County Commissioners in and for the County of Okaloosa, Florida, do hereby report that at a meeting of the Board of County Commissioners held this day, the foregoing application of Mary Jane Paul for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant has been a bona fide citizen of the State of Florida for eight years just preceding the date of this application, and that the representation made in the petition and affidavits are true, and that a pension should be granted the applicant. Paul, and that he, the said James W. Paul was, on the ____ day of May, A. D. 1917, adjudged a lunatic by a commission appointed to make investigation into the sanity of the said James W. Paul, by the county judge of Okaloosa County, Florida; that the said James W. Paul is now place in the hospital for insane persons at Chattahoochee, Florida; that the said James W. Paul is a pensioner under the laws of the State of Florida; that she, affiant, is dependant upon her said husband for support; that there are no minor children of the said James W. Paul, and the money she, affiant, is entitled to receive as the wife of said James W. Paul, if collected by her, will be spent and used solely for her maintenance, she being the only person dependant on him for support. J. Hart, D. R. Moore, all of whom I am well acquainted with, and each having been by me first duly sworn, says: That they, the affiants, were appointed by the county judge of Okaloosa County, Florida, as a commission to inquire into the mental condition of one James W. Paul; that on the ____ day of May, A. D. 1917, we made such inquiry into the mental condition of said James W. Paul; that we decided that said James W. Paul was a lunatic, and that he, the said James W. Paul was so declared by us, and he is now in the Hospital for the Insane at Chattahoochee, Florida, so we are informed and believe. Affiants further say under oath, that they are well acquainted with the family of the said James W.
