D. One Thousand Nine Hundred and twenty six, personally appeared before me a Notary Public in and for the County and State aforesaid Bulah Wright a resident of Laurel Hill Rt. 4 County of Okaloosa State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of the General Pension Laws of Florida. That she is the lawful widow of Wm. P. Wright. For proof of husband's war service, refer to Pension No. 8530 filed in the Comptroller's Office. That she was lawfully married to said Wm. P. Wright under the name of Bulah Henderson in the County of Santa Rosa State of Fla. on the 8 day of June, 1905, and that she was not divorced from him before nor has she remarried since his death, which occurred on the 12 day of March, 1926, in the County of Okaloosa, State of Fla. except as hereinafter stated. That she is a resident of Okaloosa County, Florida, and has continuously resided in the State of Florida since the last 40 years. That she was heretofore granted a pension from the State of Florida under Certificate No. ____. That she is not a pensioner of any other State. H. Fowler hath applied to the judge of the County Judge's Court of said County of Okaloosa to be appointed Guardian of Mrs. Bulah Wright, an Incompetent, and it appearing to the Judge of said Court that said Mrs. Bulah Wright, has been adjudged insane and a Guardian is necessary to look after her estate and it also appearing to the Judge of said Court that said J. H. Fowler is a fit and proper person to receive said appointment, and the said J. H. Fowler having in due form of law taken the oath and performed all other acts necessary to his just qualification as such Guardian: Now, therefore, know ye, that I, ____ County Judge in and for the County aforesaid, by virtue of the power and authority by law in me vested, do hereby declare the said J. H. Fowler duly qualified under and by virtue of the laws of said State, to act as Guardian of said Mrs. Bulah Wright, An Incompetent, and hath power, by virtue of those presents, to have the care, custody and control of said Incompetent and duly entitled to take possession of and to have and to hold, for the benefit of said Incompetent all and singular the goods, chattels, credits and estate of said Incompetent, as shall be in accordance with law, during the legal continuance of said guardianship, until the said Incompetent shall arrive at the age of twenty-one years, or until the said guardianship shall be duly revoked according to law. This cause coming on to be heard upon the bill of complaint of Mrs. Beulah Wright, an insane person, by W. H. Henderson, nephew of Mrs. Wright and next friend, and the State Florida being represented by Purl G. Adams, State's Attorney for the First Judicial Circuit of Florida, for the purpose of this hearing and the said Mrs. Beulah Wright being present in personal and represented by counsel and the complainant having produced as witnesses, Frank Clary, W. H. Henderson, I. J. Stephens, Elizabeth Brown, Mrs. Fay Belle Steele, who being duly sworn testified to the mental and physical condition and fitness of the said Mrs. Beulah Wright to manage her own affairs, and be restored to his personal liberty and complainant having produced affidavits of neighbors, and affidavit of Dr. S. E. Stephens who is a licensed and practicing physician of Okaloosa County, Florida, attesting the sanity and physical fitness of the said Mrs. Beulah Wright, and the Court having considered the same; IT IS HEREBY ORDERED, ADJUDGED AND DECREED, that the said Mrs. Beulah Wright is of sound mind judicially and is capable of managing her own affairs; that the said Mrs. Beulah Wright be immediately restored to her personal liberty; that the said Mrs. Beulah Wright be decreed to be capable mentally of attending to her own affairs as though she had never been adjudged insane.
