D. One Thousand Nine Hundred and twenty, personally appeared before me, a notary public in and for the County and State aforesaid Emily E. Cooley a resident of Okeechobee County (Fort Drum) State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of Chapter 7259, Laws of Florida, approved June 7, 1917. That she is the lawful widow of J. W. Cooley who enlisted under the name of James W. Cooley on the 15th day of July, 1862, in Company E (Dukes) Ky. Cav. C. S. A. Regiment of the State of ____ and who was honorably discharged at Camp Douglas, Chicago, Ill., May 9, 1865, on account of termination war, etc. That she was lawfully married to the said James W. Cooley under the name of ____ in the County of Hillsborough State of Florida on the 16th day of September, 1884, and that she was not divorced from him before nor has she remarried since his death, which occurred on the 22 day of April, 1903, in the County of Palm Beach, State of Fla. except as hereinafter stated. That she is a resident of Okeechobee County, Florida, and has continuously resided in the State of Florida since the 18th day of May, 1862. That she was never heretofore granted a pension from the State of Florida under Certificate No. ____. That she is not a pensioner of any other State.
