One thousand nine hundred and nine personally appeared before me, a Clerk Circuit Court in and for the county and State aforesaid, Hiram Beasley who, being duly sworn according to law, declares that he is 64 years of age, Having been born on the--day of February, 1845, in the county of Ware, in the state of GA. That he is a bona fide citizen of the county of Orange, state of Florida. That he has resided in the State of Florida continuous, since the 25th day of December, 1867. That he is the identical person who enlisted at Sanderson, Fl., under the name of Hiram Beasley, on the --day of ----------,1861, in Company D, Regiment First-Regt in the State of Florida Calvary in the service of the Confederate States (Here claim whether the service claimed was in the Confederate Stares Army or in the service of a State) and who was Honorably discharged at Sanderson, Fla., in the State of Fla., on the --day of -------, 1865, on account of Close of the War. (Here state fully any other military service performed by the applicant.) My service was in the Western Army __________ in Kentucky & Tenn___where. I was captured at Missionary Ridge, Tenn in 1864 and remained a prisoner until the close of the war. (Here give date and place of capture, imprisonment, exchange or parole) _________________________________________________________________________ That I served faithfully until Honorably Discharged from the service of the Confederate States in the year 1865, and did not desert the service of the Confederate States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. Sworn and subscribed before me, this---------day of ----------, A.D. 19-- ___________________ Physician At a meeting of the State Board of Pensions held July 10th, 1907, at which the Governor, Comptroller and Treasurer were present, the following resolution was adopted: Resolved: That persons entitled to Pensions under the Laws of Florida, who apply for the amount allowed in cases of total disability, must submit the affidavit of a reputable physician stating specifically the personal ailment and conditions that render the applicant entirely helpless and incapacitated, physically or mentally, for any work or business. The above Physician's Affidavit was blank. ************************************************************************** Page 6 SOLDIER'S APPLICATION FOR PENSION. State of Florida, Orange, County, On thiis 4 th day of November, 1903 personally appeared before me, Clerk of the Circuit Court, in and for said county and State, Hiram Beasley who being by me duly sworn, declare he is the identical person who enlisted on or about the 18 & 19 day of October, 1861, in Captain John Harvey's Company, county of Baker, in the State of Florida, and that while in actual service in said company, D of the First Regiment, of the State of Florida, and in line of duty as such soldier, at Missionary Ridge, State of Tennessee on or about --- day of Feb, 1864 (handwritten) was captured and confined in Rock Island, Illinois, Federal Prison until a few days before close of the Civil War. When he was sent home on account of sickness. That before he rejoined his command, being detained at house by sickness on account of his long confinement in prison, (about 15 months) the war was closed. This on account of physical disabilities. I am now unable to gain a livelihood by manual labor; that I have continuously since January 1st, 1885, been a citizen of Florida; that I do not own real and personal property to the value of $800 in this or any other State, and have not purposely disposed of my property for the purpose of availing myself of the provisions of the pension laws of Florida; that I am not otherwise enabled, or in a position to earn, and I have no income from any source sufficient for a livelihood by manual labor, and that I never deserted the Confederate service, and that I receive a pension from no other source. That I am a widower. Revised Statutes of Florida, Chapter II, Article I. 2077. The children of parents who are unable to support themselves, shall be required to make provision for their support. 2078. On the information filed before the Justice of the Peace of the proper district by any person whomsoever, stating that certain persons have made no adequate provision for their father and mother, or either of them, the Justice shall cause a summons to be issued to said parties and evidence to be taken as to the truth of the facts stated in the information, and if the same shall be found true, after a fair trial in which the defendents shall have the right to be heard by counsel, the Justice shall issue an order making an assessment on the children for such amount as shall be necessary for the support of their parents. 2079.
