That she is the lawful widow of Daniel M. Vaden who enlisted under the name of Daniel M. Vaden on the May day of ………, 1862, in Company Dabney’s H Artillery Branch’s Batallion Regiment of the State of Virginia and who was honorably discharged at Petersburg, 1864, on account of fever (Here give complete statement of other service, if any.) Volunteers …….. Brigade, and was honorably discharged on the ……. .day of ……….. 1864; that while engaged in such military service, and in line of duty in the State of Va. on the ….day of August, 1863, he was disabled or wounded as follows: contracted fever in Bismal Swamp Va., was sent to the Hospital at Petersburg Va. and remained in said Hospital two months; the fever resulted in rheumatism, this rheumatism became chronic and was not able to do but little service after this sickness until the war ended. This rheumatism has followed me over since and as I grow older the disease gets worse, and I am not able to do manual labor. Where was command surrendered: At Greensborough N. C.
