This statement of service is furnished under the provisions of the act of Congress approved July 11, 1919. 2. This statement is furnished primarily for historical and statistical purposes, although it may be used in adjudication claims in which the State if solely concerned. It is not to be used before a court of Justice or in support of any claim against the Federal Government. The law prohibits the furnishing of any information that may be used in the prosecution of a claim against the Government, except to the proper Government officials. 3. Applications from individuals, other than historians and statisticians, for information from this statement of service should be denied and the applicant directed to apply for the information to The Adjutant General of the Army, who is the custodian of the official records of the Armies of the Untied States. Exceptions to this rule may be made in the case of officials of patriotic and philanthropic societies and associations when it is known positively that the information is to be used exclusively by said society or association, and will not be made public nor communicated to any individual who may use it directly or indirectly as a basis for, or in the prosecution of, a claim against the Government, or to the injury of the soldier. 4. Except as hereinbefore provided, all persons seeking information relative to the military and medical histories of the soldier should be denied information from or access to, this statement of service, as otherwise information legally unobtainable would be made available to persons who might perpetrate fraud upon the Government, or who might seek to injure the soldier. 5. Except the data contained on the first four lines and that relating to battle causalities and physical disability, this statement is prepared as far as practicable from the service record, and no effort has been made to compare the date obtained from the service record with other records, except where an error or discrepancy is patent. 6. Some enlistment papers show age at enlistment, while others show date of birth. The entries recorded herein are in the same form as those on the enlistment paper.
