A. DECLARATION FOR ORIGINAL INVALID PENSION. A. To be executed before a court of record or some officer thereof having custody of its seal. State of Florida County of Pasco On this 21 day of August, A.D. one thousand eight hundred and eighty- eight personally appeared before me, a Notary of the Public for the State, a court of record within and for the county and State aforesaid, Caleb Griffin, aged 71 years, a resident of the ______ of Florida county of Sumpter State of Florida, who, being duly sworn according to law, declares that he is the identical Caleb Griffin, who was ENROLLED on the the later part day of 1836, 1836, in company of the _____ regiment of Florida Infintry commanded by Capt William Reed, who was honorably DISCHARGED at at the lower Springs in Columbi Co on the sometime in Oct day of Oct, 1836, that his personal description is as follows: Age, 71 years; height, five feet 10 inches; complexion, fair; hair, Brown, eyes, Blue. That while a member of the organization aforesaid, in the serive and in the line of his duty at lower Springs, in the State of Florida, on or about the 25 day of Oct, 1836, he recived asma from a spell of sickness in line of duty and has never bin well since am intirly disabled from manuel labor from said dissease It has bin so long don’t recolect the date I have lost all my papers and all so lost my discharge I was attended by Dr. Spence and he is ded and I dont the zack dates of treatment. That he was treated in hospitals as follows: In no hospitle. Since I was discharged my family Dr. who attended for said asma is Dr. Snow of Sumpterville. That he has never been employed in the military or naval service otherwise that as stated above the asma taken me in the year of 1836 and Intirly Disabled to preform manual labor. That since leaving the service this applicant has resided in the State of Florida in the State of Florida, and his occupation has been that of a Farmer That prior to his entry into the service above named he was a man of good, sound physical health, being when enrolled a Private. That he is now Intirley disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension-roll of the United States. He hereby appoints Wilksom Call of Washington, State of D.C., his true and lawful attorney to prosecute his claim. That he has never received no applied for a pension.
