WIDOW’S PENSION CLAIM UNDER THE ACT OF 1909 (Form A) STATE OF FLORIDA COUNTY OF SUWANNEE On this 9 day of January, A.D., One Thousand Nine Hundred and Twelve, personally appeared before me a Notary Public in and for the County and State aforesaid Elminor E. Adams, a resident of Wilmarth County of Suwannee, State of Florida, who being duly sworn according to law, makes the following declaration in order to obtain a pension under the provisions of Chapter 5885, Laws of Florida, approved June 7, 1909. That she is the widow of James M. Adams who was enlisted under the name of James M. Adams, on the 13th day of May, 1862 in Company C, 8th Infantry Regiment of the State of Florida, and who was honorably discharged at City Point, Virginia, April 12, 1865, on account of close of the war. That he also serviced for proof of husband’s service see approved pension claim No. 1425 on file in the Comptroller’s Office. That she was lawfully married to the said James M. Adams under the name of Elminor E. Grant in the County of Lafayette, State of Florida, on the 8th day of March, 1860 and that she was not divorced from him, and has not remarried since his death, which occurred on the 13 day of December, 1911, in the County of Suwannee, State of Florida. That she is a resident of Suwannee County, Florida, and has continuously resided in the State of Florida since the don’t remember day of month, January, 1853. ************************************************************************ That she does not own property, including real estate, personal property, mortgages or other collateral securities, stocks or bonds in this or any other State to exceed the value the sum of Five Thousand Dollars. That he is a bona fide citizen of the county of Suwannee, State of Florida. That he has resided in the State of Florida continuously since the 1st day of January, 1851. That he is the identical person who enlisted at Houston under the name of James M. Adams, on the 13th day of ¬¬¬¬¬¬¬¬¬¬¬¬¬May, 1862, in Company C, Regiment 8th, the State of Florida, in the service of the Confederate States Army and who was HONORABLY DISCHARGED at City Point, in the State of Virginia, on the 12th day of April, 1865, on account of surrender of General Lee. No other military service performed except with regiment named above. Captured near Appomattox Court House, on 9th day of April, 1865. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States Army in the year 1865, and did not desert the service of the Confederate States Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. That I was at or near Appomattox Court House on the day of surrender and was captured the same day and taken to City Point, Virginia. That I do not own property, including real estate, personal property, mortgages or other collateral securities, stocks or bonds in this or any other State to exceed the value the sum of Five Thousand Dollars. Goodbred, who being duly sworn, deposes and says that he is a physician; that he is a resident of the State and County aforesaid; that he personally knows James M. Adams, the applicant named in the foregoing application for a pension. The deponents further say that they have carefully examined the said applicant’s physical condition and find: (Here state nature, character and extent of wounds, disease or disability. Please avoid technical terms.) That he has chronic naso-Pharangeal catarrh, which has involved the ears and eyes. There is a perforation of right side of nose resulting apparently from removal of cancer, and he now has a cancer about the center of left cheek, about half an inch in diameter. This deponent further says that the said James M. Adams is permanently partially disabled by reason of such diseases from earning a livelihood for himself by manual labor. Dear Mrs. Mathews: Re: James Madison Adams Replying to your letter of April 27th, I wish to advise that the records in this office show that James Madison Adams received a pension from the State of Florida and he stated in his application for pension that he enlisted at Houston, Fla., on May 13, 1862, in Company C, 8th, (Capt. David Lang’s) Regiment, Florida Infantry, Confederate States Army, and served until discharged at City Point, Va. Where he was discharged on account of the close of the war. The records also show that he was born on Sept. 4, 1837, that he was married to Elminor E. Grant on March 8, 1860, and died on the 13th day of December, 1911. Adams, whose pension application you will find inclosed, lives just across the river in Suwannee County. There is no officer authorized to take acknowledgments in his neighborhood, so he came to Mayo the nearest point for the purpose of making out his claim. He is a well known citizen and the two citizens of Lafayette County who make oath as to Mr. Adams property valuation are in position to know, and do know the amount of property owned by this applicant.
