That he is a bona fide citizen of the county of Suwanee (sic), State of Florida. That he has resided in the State of Florida continuously since the 10 day of Sept., 1879. That he is the identical person who enlisted at Teboville Ga., under the name of Andrew J. Agee, on the 15 day of Oct., 1861, in Company K, Regiment 26th of the State of Ga., in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate State (sic) and who was honorably discharged at Point Look Out in the State of Virginia about on the 1 day of July, 1865, on account of war being ended and let loose from prison.
