That he is the identical person who enlisted at Gainesville, Fla., under the name of W. T. Beach, on the 17 day of May, 1862, in Company F, Regiment 2nd Fla of the State of Fla in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Beach, private, Company F, 2d Florida Cavalry, Confederate States Army, enlisted May 17, 1862. The roll of the company for May and June, 1864, last roll on file, shows him “Dismounted Feb. 10/64.” No later record of him has been found. F. C.
