Widow's Pension Claim
Under General Pension Law Of Florida As Amended By Acts Of 1921

On this 3rd day of November, A. D. One Thousand Nine Hundred and twenty-four, 
personally appeared before me, a Clerk Circuit Court in and for the County and 
State aforesaid Elizabeth Addison a resident of Raiford County of Union State of 
Florida, who being duly sworn according to law, makes the following declaration in 
order to obtain a pension under the provisions of the General Pension Laws of 
Florida as amended by Chapter 8400, Acts of 1921.

That she is the lawful widow of John W. Addison. 

(State here if husband drew a pension, and when.)

Do not know exact date but had been drawing twenty-five or thirty years.

That she was lawfully married to the said John W. Addison under the name of 
Elizabeth Anderson in the County of Bradford State of Florida on or about the ____ 
day of November, 1885, and that she was not divorced from him before nor has she 
remarried since his death, which occurred on the 26th day of October, 1924, in the 
County of Union, State of Florida except hereinafter stated.

That she is a resident of Union County, Florida, and has continuously resided in 
the State of Florida since the 17th day of February 1870, date of birth.

That she was heretofore granted a pension from the State of Florida under Certificate 
No. ____.  That she is not a pensioner of any other State.

That her Postoffice address is Raiford, County of Union State of Florida.

certify that the above declaration, etc., was made known and fully explained to the 
applicant before swearing, and that I have no interest, direct or indirect, in the 
prosecution of this claim.

personally appeared G. W. Anderson and E. P. Johnson, who, being duly sworn according 
to law, deposes and says each for himself that he was present at the marriage of 
John W. Addison and Elizabeth Anderson which occurred at Raiford on or about the 
20th day of November, A. D. 1885, in the County of Bradford, State of Florida, and 
that the ceremony was performed by Joseph L. Hill, J. P.

Report of County Commissioners

We, the undersigned, County Commissioners in and for the County of Union, Florida, 
do hereby report that at a meeting of the Board of County Commissioners held this 
day, the foregoing application of Elizabeth Addison for a pension under the Laws of 
Florida, was investigated by us; that we are satisfied that the applicant has been
a bona fide citizen of the State of Florida for eight years just preceding the date
of this application, and that the representation made in the petition and affidavits
are true, and that a pension should be granted the applicant.

Witness our hands this 3 day of October, A. D. 1924.
