Richard's Company, county of Bradford, in the State of Florida, and that while in actual service in said company, A 10th Fla. Infy. Regiment, State of Florida, and in line of duty as such soldier, at Petersburg, State of Virginia on or about the 30th day of July, 1864, I was wounded and was honorably discharged at the expiration of my service on the ____ day of ____ in ____ county, State ____, I was wounded at Petersburg, Virginia by the ? Confederate ? on the 30th day of July 1864, at this place my left ankle was broken and crushed causing the amputation of my left leg just below left knee joint and that as the direct result of said injuries, thus received in line of duty during the war, I am now unable to gain a livelihood by manual labor; that I have continuously since January 1st, 1883, been a citizen of Florida; that neither I, nor my wife, nor both combined, own real and personal property to the value of $800 in this or any other State, and have not purposely disposed of our property for the purpose of availing ourselves of the provisions of the pension laws of Florida; that I am not otherwise enabled, or in a position to earn, and I have no income from any source sufficient for a livelihood by manual labor, and that I never deserted the Confederate service, and that I receive a pension from no other source. D. One Thousand Nine Hundred and nine personally appeared before me, a Clerk Circuit Court in and for the county and State aforesaid, John W. Addison who, being duly sworn according to law, declares that he is 63 years of age, having been born on the 18th day of April, 1846 in the county of Bradford, in the State of Florida. That he is a bona fide citizen of the county of Bradford, State of Florida. That he has resided in the State of Florida continuously since the 18th day of April, 1846. That he is the identical person who enlisted at Baldwin, Florida, under the name of John W. Addison, on the ____ day of April 1864, in Company "A" 10th Fla. Inft. of the State of Florida in the service of the Confederate States Army and who was HONORABLY DISCHARGED at being wounded, mustered out April 9th 1865, in the State of Virginia, on the 9th day of April, 1865 on account of termination of the War. That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States Army in the year 1865, and did not desert the service of the Confederate States Army nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was at home, loosing my left leg just below knee joint, same being amputated before coming home, in hospital at Petersburg, wound received was in the battle of Petersburg. That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, nor does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. Addison Pensioner No. 1219 of the State of Florida under the laws of Florida do hereby make application to the State Board of Pensions for an increase in pension allowed me at the rate of $150.00 per annum. I am unable on account of the disabilities shown below and by attached affidavit of a reputable physician, to earn a livelihood by manual labor. (Here state fully and plainly the disability from which the applicant for increase is suffering, state any loss of limbs or eyesight and give age. Please avoid the use of technical terms.) Please avoid technical terms.) That his left leg is off above ankle - from wound received at Battle of Petersburg, Va. Applicant is 67 years old. This deponent further says that the said John W. Addison, and have known him for the past thirty-five years. We are also familiar with his physical condition and know of our own knowledge that he has lost one foot and have every reason to believe that such loss was incurred while in actual military service during the civil war. Park Avenue Valdosta, Georgia Re: Nathaniel Encok Roberts & James William Addison, Confederate Veterans Dear Mr. Addison: Your letter of February 5, 1965, requesting Confederate Military Service Record of the above subject captioned veteran, is acknowledged. A solider by the name of Nathan Roberts was given a Confederate Pension under Certificate #5539, in this application he set forth the following information. He was born in Ware County, Georgia, January 23, 1823, enlisted 1862 in Lake City, Florida, in Company E, 9th Florida Regiment, Finnegan's Brigade, and was discharged April 9, 1865, at Appomattox, Virginia. There is no record if this veteran was married or of his death. Enclosed is also photostatic copy of sheet #215 from "Soldiers of Florida" Roll of Company E - 9th Florida Infantry, indicating Nathan Roberts as mustered out April 9, 1865. A John W. Addison received a Confederate Pension on Pension Certificate #2540, this application originating from Bradford County, the veteran stated he was born October 18, 1846, in Bradford County, Florida, enlisted in Baldwin County, Florida, April, 1864, in Company A - 10th Florida Infantry, and mustered out April 9, 1865, in Virginia at the close of the war, and that he died October 26, 1924, in Union County, Florida. It is indicated he was married to Elizabeth Anderson November, 1885, in Bradford County, Florida. I am enclosing photostatic copy of sheet #220 from "Soldiers of Florida" on which the name of John W. Addison appears on the roster roll of Company A- 10th Florida Infantry, as being mustered in April, 1864, and mustered out April 9, 1865; it is also indicated he was wounded at Petersburg, Virginia, July 30, 1864. It is certified that the above information and photostatic documents attached are from official records on file in the State Board of Pensions, Office of the State Comptroller, Tallahassee, Florida.
