That he is a bona fide citizen of the county of Walton, State of Florida. That he has resided in the State of Florida continuously since the………day of February, 1874. That he is the identical person who enlisted at Greenville, Ala, under the name of James Adkison, on the……………day of March, 1862, in Company “A”, Regiment 33rd Ala., of the State of Alabama in the service of the (Here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate States Army and who was honorably discharged at Macon, in the State of Georgia, on the…………..day of April, 1865, on account of close of the war. (Here state fully any other military service performed by the applicant.) Never rendered any other service than in Confederate States Army. I was captured by Sherman’s Army at Columbus, Ga., 12 days before peace was declared and was released at Eufaula, Ala., after the surrender. Regiment, of the State of Alabama, and in line of duty as such soldier, at Chickamauga, Georgia, on or about the 19th day of September, 1864, and was honorably discharged at the expiration of my service on the 28th day of May 1865 in Andersonville, Sumter County, State of Ga., (Here state fully and clearly all the facts, showing the injury, its character, and especially the extent of the injury and disability resulting therefrom.) I received a gunshot wound in the Rite (sic) leg one inch above the knee breaking the bone and on or about the 22nd day of July, 1864, at the Battle of New Hope Church in the State of Ga., I received a gunshot wound to the left leg about one inch above the ankle fracturing the bone and have continuously ever since give me much pain and trouble. And I am also suffering from impaired vision and general disability, and that as the direct result of said injuries, thus received in line of duty during the war, I am now unable to gain a livelihood by manual labor; that I have continuously since January 1st, 1885, been a citizen of Florida; that neither I, nor my wife, nor both combined, own real and personal property to the value of $800 in this or any other State, and have not purposely disposed of our property for the purpose of availing ourselves of the provisions of the pension laws of Florida; that I am not otherwise enabled, or in a position to earn, and have no income from any source sufficient for a livelihood by manual labor, and that I never deserted the Confederate service, and that I receive a pension from no other source. That on December 31, 1908 he was thrown from his wagon and had his hip joint dis-located and the thigh fractured near the hip joint and his right eye injured and that at the present time on account of said injuries he (sic) rendered wholly unable from earning a livelihood for himself by manual labor, being unable to get about except with the aid of crutches.
