Report of the County Commissioners: "We the undersigned, County Commissioners in and for the County of Walton, FL, do hereby report that at a meeting of the Board of County Commissioners held this day, the foregoing application of John Stafford for a pension under the Laws of Florida, was investigated by us; that we are satisfied that the applicant does not own property (including the property of his wife) to exceed the value of $5,000 and that the representations made in the petition and affidavits are true, and that a pension should be granted to the applicant. Attest: Chas. H. Gordon, Clerk Circuit Court 2. Soldier's Pension Claim Under the Act of 1909 (Form A.) State of Florida, County of Walton On this 15 day of July A.D. One Thousand Nine Hundred and nine personally appeared before me, a Notary Public in and for the county and State aforesaid, John Stafford who, being duly sworn according to law, declares that he is 75 years of age, having been born on the 26 day of Nov, 1833, in the county of "Don't remember county" in the state of Georgia. That he is a bona fide citizen of the county of Walton, State of Florida. That he has resided in the State of Florida continuously since the ___day of October, 1865. That he is the identical person who enlisted at Geneva, Ala. under the name of John Stafford on or about the 15 day of May, 1863, in Company K Regiment, Clanton's Brigade, the State of Alabama, in the service of the Confederate States Army and who was honorably discharged at Montgomery in the State of Alabama, on the ____day of May, 1865, on account of close of war. [Other military service performed by applicant: "None except in Confederate Army".] That I served faithfully until HONORABLY DISCHARGED from the service of the Confederate States in the year 1865 and did not desert the service of the Confederate States nor take the oath of allegiance to the United States until after the surrender of the Confederate Armies. (Here state whereabouts at close of Civil War.) That I was "given a furlough in the 26 day of Feb. 1865 for 30 days and shortly after getting home two of my company came by my home and left me word not to return as the company was captured and we had instructions to meet in Montgomery, Ala., and in May we took the oath of allegience there." State of Florida, County of Walton We, the undersigned citizens of Walton County, State of Florida, do hereby certify that we personally know John Stafford, who is an applicant for a pension under the laws of Florida, and that from our own personal knowledge, and from the best information available, we believe that the applicant does not own property (including the property of his wife) to exceed in value the sum of $5,000, and that the statements made by him relative to the value of his property are true and correct. Physician's Affidavit State of Florida, County of Walton Before me personally came John F. King, who being duly sworn, deposes and says that he is a physician; that he is a resident of the State and County aforesaid; that he personally know John Stafford, the applicant named in the foregoing application for a pension. This deponent further says that he has carefully examined the said applicant's physical condition and finds: (Here state nature, character and extent of wounds, disease or disability. Please avoid technical terms.)"that he has a scar the entrance of a ball at the lower part of the mastoid _rouss of temporal bone passing out at center of nose blinding the left eye; there is a consistent discharge of pus from the wound and almost total disability." This deponent further says that the said John Stafford is permanently partially disabled by reason of such injury and age from earning a livelihood for himself by manual labor. (Please note carefully resolution below before certifying to total disability.) Sworn and subscribed before me, this 15 day of July, A.D. 1909, J. F. King, M. D. (Physician) James A. McLean, Notary Public At a meeting of the Sate Board of Pensions held July 10th, 1907, at which the Governor, Comptroller and Treasurer were present, the following resolution was adopted: "Resolved: That persons entitled to Pensions under the Laws of Florida, who apply for the amount allowed in cases of total disability, must submit the affidavit of a reputable physician stating specifically the personal ailment and conditions that render the applicant entirely helpless and incapacitated, physically or mentally, for any work or business." ************************************************************************************ [Affidavit of property owned] That I do not own property, including real estate, personal property, stocks, bonds, mortgages or other collateral securities of any kind in this or any other State, no does my wife own with me jointly or separately, property to exceed in value the sum of five thousand dollars. That the following is a true and correct statement of all the property owned by me or by my wife, jointly and separately in this or any other State: Real estate located at 120 acres in Section 6 Township 2 N, Range 19 West, Walton County. . . . . . . . . . . . . . . . $300.00 Cattle, horses and other live stock . $ 40.00 Total . . . . . . . . . . . . . . . . $390.00 That I have heretofore been granted a pension from the State of Florida under pension certificate No. 1 at the rate of $120. per annum. (Here state any disabilities, physical or mental.) "I was struck with a merinic? ball about lower point of left ear and came out through center of my nose fracturing my face bone from the effects of which wound my head has ever since continued to run corrupt matter which also caused the loss of left eye.
