One Thousand Nine Hundred and Seventeen personally appeared before me, a Notary Public in and for the county and State aforesaid, Henry Adkinson, who, being duly sworn according to law, declares that he is 76 years of age, having been born on the 19 day of May, 1841, in the county of Macon, in the State of Georgia. That he is a bona fide citizen of the county of Washington, State of Florida. That he has resided in the State of Florida continuously since the 24 day of December, 1904. That he is the identical person who enlisted at Hanson’s old Field or a Gofer Hill Ga, under the name of Henry Adkinson, on or about the 1st day of June, 1862, in Company I or G, Regiment Fifth of the State of Georgia in the service of the (here state whether the service claimed was in the Confederate States Army or in the service of a State.) Confederate States Army and who was honorably discharged at Richmond, in the State of Va., on or about the 20 day of May, 1865, on account of close of the war.
