"This is to certify that on the day and date above written we the undersigned 
met by request with the church at Mt. Pleasat in Carroll County and after 
Religious Services proceeded as Presbytry to examine the churches membership 
in regard to principles of faith and practice as held and maintained by the 
Missionary Baptist Church and finding the same Orthodox and regularly 
dismissed from church of the same faith and order did further proceed to 
constitute and organize said membership into a Missionary Baptist Church known 
as Mount Pleasant Church.
