Cook, a private, of Captain D.B. Langston's Company K, of the 3rd GA, Regiment of Confederate States Infanty, was enlisted by Lieut D. V. B. Langston, of the 3rd Ga Regiment of Infanty at Athens, Georgia on the 11th day of June 1861, to serve one year; he was born in Clarke [Co] in the State of Georgia is 19 years of age, 5 feet, 8 inches high, florid complexion, light eyes, light hair, and by occupation when enlisted a Farmer. During the last Five months said soldier has been unfit for duty 150 days. This soldier was wounded at Chancellorsville, may 3, 1863 and from that date until now has been under medical treatment. He cannot perform the duties of a soldier on account of his incapacity to eat and want of strength. Station: Camp Fear Boundry Station Date: Oct 26, 1863 D.B. Langston, Capt I certify that I have carefully examined the said Private Wm M. Cook of Capt D.B. Langton's Company and find him incapable of performing his duties of a solder because a laceration woud of lower jaw, received at the Battle of Chancellorsville. Nisbet, Lt Col Additional Comments: Note: Cook was born January 20, 1844 in Clarke County and died January 30, 1938 in Washita County, OK.
