What is your name and where do you reside? (give State, County and Post office) M J Richardson, Livingston Floyd Co. GA. 2. Where did you reside on Jan 1st, 1894 and how long have you been a resident of this State? Floyd ever since 1845 3. When and where were you born? born in Coweta Co GA in 1834 4. When and where and in what company and regiment did you enlist or serve? enlisted in Co D 29 Ga Regiment at Rome Ga 5. How long did you remain in such company and regiment? 3 years or more 6. For how long a period did you discharge regular military duty? 3 years or more 7. When, where and under what circumstances were you discharged from service? paroled in 1865 at Kingston Ga. 8. H. H. Camp, of said State and County, having been presented as a witness in support of the application of M. J. Richardson for pension under the Act approved December 15th, 1894, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your name and where do you reside? W F Carroll Livingston Ga. W. H. H. Camp Livingston Ga. 2. Are you acquainted with M. J. Richardson, the applicant; if so how long have you known him? 40 years 3. Where does he reside, and how long and since when has he been a resident of this state? Livingston Ga. All his life 4. Do you know of his having served in the Confederate army or The Georgia militia? How do you know this? Confederate Army I served with him 5. When, where and in what company and regiment did he enlist? enlisted in Co D 29 regiment; at Rome Ga 6. Were you a member of the same company and regiment? yes 7. How long did he perform regular military duty, and what do you know of his service as a Confederate Soldier, and the time and circumstances of his discharge from service? 3 years or more, don't know, Good Soldier 8. What property, effects or income has the applicant? (Give your means of knowledge.) none 9. What property, effects or income did the applicant possess in 1895 and 1896, and what disposition, if any, did he make of same? none 10. What is the applicant's occupation and physical condition? Richardson, Applicant for Pension in case herewith, who moves to amend his application by way of supplement and says on oath the following facts are true as to his ability to labor at any work or calling. & as to his financial standing. That on account of his age and infirmities he is not able to labor at any work or calling sufficient to earn a support; his infirmities are old age, rheumatism and tumor on back which render him unable to do any kind of labor and that he has had no property of any kind for 3 years but that he had given in $25.00 taxes, as agent for wife, household kitchen furniture & one cow. The only opportunity he has to make a living is little jobs shucking corn or such like. That in the original application he stated that he could earn $8.00 per month, but in fact he was in 1895 working with a man for a living & considered his board worth that amount, but that now he can't get a home like that or any work to do because he is not able to work . Camp. well know to said officer as being of trustworthy character, & entitled to full faith & credit who being duly sworn says on oath that he served with M. J. Richardsonson in the Army and he lives neighbor to him now and he knows of his own knowledge that the facts as stated in his affidavit hereto attached are true W H H Camp Lieut Co D 29 Ga Sword to & subscribed before me Dec 21st 1897 John P Davis Ordinary ------------------------------------------------------------------------------------ ----------------------------------- AFFIDAVIT OF PHYSICIANS State of Georgia Floyd County Personally came before me D. T. McCall and W. D. Hoyt, both known to me as reputable physicians of said county, who, being severally sworn, say on oath that they have examined carefully M. J. Richardson, applicant for pension under the Act of 1894, and after such personal examination say that his precise physical condition is as follows: We find a tumor on the back near the neck 4 to 6 inches and rheumatism of the right hip and corns under his feet he is 68 years old very poorly nourished we don't think from his condition that he is capacitated to make a support we further say on oath that the physical condition of applicant renders him unable to labor at any work or calling sufficient to support himself, and that we have no interest in said pension being allowed. D. T. McCall W. D. Hoyt Sworn to and subscribed before me, this the 18th day of January. 1897. John P. Davis Ordinary. --------------------------------------------------------------------------- ORDINARY'S CERTIFICATE State of Georgia Floyd County I, John P. Davis, Ordinary in and for said county, hereby certify that the applicant M. J. Richardson resides in said County, and has been a bona fide resident of this state since the first day of January, 1894 and that the witnesses viz: D. T. McCall, W. D. Hoyt & W. F. Carroll are of trustworthy character and that their statements are entitled to full faith and credit. I further certify that before answering the foregoing questions, the applicant and each witness took the oath hereon prescribed, and that the full text of the affidavits was read to the applicant and witnesses before same was signed. I further certify that the tax digests of Floyd County show that applicant returned for taxation in his name in 1895, $25.00 dollars of property, and in 1896, $25.00 dollars of property. In my opinion, the foregoing claim is made in good faith. Witness my hand and seal of office, this 20th day of January 1897. .
