Phillips, widow of Thomas S. Phillips Affidavit to be Made by Widow State of Georgia, County of Heard In person came before the undersigned Ordinary in and for the County of Heard, Mrs. Nancy A. Phillips, who being sworn according to law says under oath that the is the widow of T.S. Phillips, who was a soldier in the service of the Confederate States and served as a member of Company I of the 13th Regiment of Alabama Volunteers; that he enlisted in said service on or about the 30th day of September 1861 and was in the Army up to the 6th day of June 1864. That while in the Army he was on the 6th day of June 1864 at Gaines Mill VA shot by a minnie ball from a stray gun in the center of the body near the region of the heart. He lived about three quarters of an hour and died there. Deponent further swears that she was the wife of the deceased soldier during his term of service in the Army and that she has never married since his death; that she became his wife on the 16th day of March 1856 and that she has resided in Georgia continuously since the 17th day of November 1882; that Georgia is her home and was such on the 23rd day of December 1890 and since said date she has not lived in any other state or locality. Deponent being the widow of said deceased soldier husband, applies for the pension provided by the Act of the General Assembly of Georgia approved December 23, 1890, for the pension year ending February 15, 1892 and herewith tenders the proof of her right to receive the allowance granted by said Act. Crain, Ordinary Affidavit of Three Witnesses State of Alabama, County of Randolph In person came before the undersigned Judge of Probate in and for said County, witnsses Martin P. Pittman, John B. Carlisle and Thos. J. East, each known to the attesting officer as truthful, reliable and reputable witnesses who severally say under oath that from their own personal knowledge, Mrs. Nancy A. Phillips of the County of Heard, State of Georgia, is the widow of T.S. Phillips who was a soldier in Company I of the 13th Regiment of Alabama Volunteers. That said soldier enlisted in the service of the Confederate States on or about the 30th day of September 1861. That while ins aid service or by reason of said service in the Army, he lost his life as follows: He secured a gun shot wound near Gainsville on or about the 6th day of June 1864 from which he died on the field. The above witnesses were all members of the same Company and M.P.Pittman was present and saw him secure the wound. We further swear that Mrs. Nancy A. Phillips was the wife of said soldier during the service and that she has not intermarried since his death and that she resides in Heard County in the State of Georgia. M.P. Pittman, J.B. Crain, Ordinary in and for said County of Heard, State of Georgia, hereby certify that I am acquainted with Mrs. Nancy A. Phillips, the applicant for pension in this case and know from my own knowledge or from positive proof presented to me by reputable witnesses that she resides in this County and that she resided in the State of Georgia on December 23, 1890 and has not lived out of the State since that date. That she is the widow of Thos. A. Phillips, deceased, and as such has heretofore been allowed a pension for the year ending February 15, 1892. In witness whereof, I have hereunto set my hand and affixed the seal of my office this the 17th day of January 1893. G.A. Crain, Ordinary State of Georgia, Heard County Personally came before the undersigned Ordinary in and for said County, witnesses Julia A. Prince and Mary A. Thompson, each known to the attesting officer as truthful witnesses entitled to full faith and credit as such and who being d uly sworn say on oath that of their own personal knowledge, Mrs. Nancy A. Thompson Phillips (note: Thompson is lined through but legible) widow of Thos. S. Phillips is a native of the State of Georgia. That she was born and raised in Fayette County, Georgia and that we were born and raised in the same neighborhood with Mrs. Nancy A. Philips until we were all grown. Signatures of Witnesses: Julia A. Prince (lined through and Julian E. Prince written in), Mary Thompson, X, her mark State of Georgia, County of Heard I, G.A. Crain, Ordinary in and for said County of Heard, State of Georgia, hereby certify that I am acquainted with Mrs. Nancy A. Phillips the applicant for pension in this case, and know from my own personal knowledge that she resides in this County and has not lived out of the state. That she is the widow of Thomas S. Phillips, deceased and as such has heretofore been allowed a pension for the year ending February 15th, 1894. In witness whereof I have hereunto set my hand and affixed the seal of my office this the 16th day of January 1894. G.A. Crain, Ordinary For Widow's Heretofore Allowed Pensions State of Georgia, County of Heard Personally cmes Mrs. Nancy A. Phillips who being sworn says on oath that she is a bona fide resident of said County of Heard, State of Georgia and that she has resided in said State since November 1881. That she is the widow of T.S. Phillips, who was a soldier in Company I of the 13th Regiment of Alabama Volunteers; that he lost his life on the 6 day of June 1864 as follows: near Gainsville in the State of Virginia. He was shot through the bodie and there died in 40 minutes from said wound. That she was born and raised in Fayette County, Georgia. Deponent further swears that the became his wife in the year 1856 and has been allowed a pension as a resident of Heard County for the year ending Feb 15, 1895 and now applies for the pension provided by law for the year ending Feb 15, 1896.
