PHILLIPS.......Application for Pension Due to a Deceased Pensioner (To be paid to the Ordinary for Expenses of Funeral and Last Illness) (Under Act Approved August 15, 1904) GEORGIA, MONROE COUNTY Personally before me, the Ordinary of said County, comes G. W. IVEY of said County, who after being sworn, on oath says that he know MRS. G. W. PHILLIPS of said County, and that said Pensioner was on the Pension Roll of said County at the time of death, which occurred in Monroe County, in this State, on the 2ND DAY OF OCTOBER 1925 and that A PENSION OF ONE HUNDRED TWENTY FIVE ($125.00) DOLLARS was due and unpaid at the time of pensioner's death, and that pensioner left no widow or dependent children surviving, and no estate of any value sufficient to pay these funeral expenses, which amounted to the sum of $119.00, per sworn statements fully and completely ITEMIZED hereto attached. Maddox, who after being duly sworn says on oath that he was present and witnessed the marriage ceremony between GEORGE W. PHILLIPS AND MISS SIDDY R. TINGLE; that he does not now recall the exact date, but that it was in the year 1859 or 1860. Sworn to and subscribed before me, this the 26th day of January 1915. J. H.
